摘    要:目的 探讨前列腺及其尿道经直肠超声(TRUS)测量和形态学指数方程在良性前列腺梗阻(BPO)诊断中的意义。方法 232例BPH者根据Q_(max)分BPO组89例和对照组143例,应用TRUS方法测量前列腺和移行带(TZ)的各径线、前列腺部尿道曲率。经SPSS软件处理及应用逐步判别分析方法建立BPO形态学指数(BPOMI)方程(Y_2,Y_1),计算BPOMI。结果 BPO主要相关因素为前列腺部尿道曲率、前列腺和TZ前后径、前列腺左右径(入选最小F=3.84,剔除最大F=2.71)。将上述数据分别代入指数方程Y_2和Y_1,并比较其大小,当BPOMI(Y_2/Y_1)>1时,提示BPO存在。BPOMI逐步差别方程在诊断回复结果和与临床一致性检验结果的诊断检验时,显示BPOMI与TRUS、Q_(max)的测量结果有良好的对应性。结论 BPOMI是一种新的BPO诊断方法,有一定的临床价值。

Abstract:Objective To investigate the significance of transrectal ultrasound (TRUS) and morphological index (MI) in diagnosis of benign prostate obstruction (BPO). Methods 232 cases of BPH were divided into BPO group 89, control group 143 according to Q_(max)<15 ml/s and diameter determinations of entire prostate or transition zone (TZ) and curvature of prostatic urethra by TRUS. Morphological Index (BPOMI)Equation(Y_1, Y_2) was built by using SPPS software. The data above-mentioned were put in the BPOMI equation (Y_1, Y_2) and both dimension was compared. Results When BPOMI (Y_2/Y_1)>1, BPO is suggested. BPOMI is favorable for the modality of lesions corresponding with TRUS and Q_(max). The sensitivity, specificity, coincidence rate and diagnosis index of BPOMI are 85.4%, 91.6%, 89.2% and 77.0%. Conclusions BPOMI(Y_2/Y_1) is a new method in diagnosis of BPO.
