摘    要:目的探讨自身免疫性疾病(AD s)自体造血干细胞移植(AHSCT)后感染的临床特点与相关因素。方法对36例自身免疫性疾病患者接受自体造血干细胞移植后的感染情况进行综合分析,包括感染特点、细菌培养、感染相关因素等。结果36例患者发生感染43例次,感染部位主要分布在口腔、肛周、肺部,感染病原体以革兰氏阴性杆菌、真菌、病毒为主。细菌耐药率高。结论感染的发生与患者原发病、病史、环磷酰胺用量、预处理方案及移植方式有关。细菌耐药率高与长期应用抗生素有关。

Abstract:Objective To discuss the characteristics of infection in patients with autoimmune disease after autologous hematopoietic stem cell transplantation(auto-HSCT).Methods Thirty-six cases treated with auto-HSCT were analysed.The related factors were composed of clinical characteristics of infection,primary disease,results of bacterial culture,and reciprocal relationship of infection and related factors in transplantation.Results 43 times infection had occurred in 36 patients of this group.The main sites of infection were oral cavity,anus,lungs.The causal agents were gram-negative bacillus,fungus and virus.Drug resistance of bacterial was high.Conclusion The occurrence of infection in this group had significant relationship in different kinds of primary disease,disease history,the total dosage of CTX,conditioning regiments for transplantation,and classification of transplantation.Drug resistance of bacterial was high as a result of long term administration of spectrum antibiotic.
