中药肠内营养剂干预糖耐量低减的临床观察

摘    要:目的:观察中药肠内营养剂干预糖耐量低减(impaired glucose tolerance,IGT)的临床疗效。方法:将300例IGT患者随机分为对照组和研究组,每组各150例。对照组在临床标准化诊疗方案进行降血糖、降血脂、减肥及运动治疗,研究组在对照组的基础上给予特定主食中辨证加入中药细粉组成中药肠内营养剂。比较两组患者的临床疗效及治疗前后中医证候积分变化情况,观察两组患者治疗前后FPG、餐后2 h血糖(2h blood sugar,2h PG)、糖化血红蛋白(glycosylated hemoglobin,Hb A1c)、低血糖发生率、血清三酰甘油(three acyl glycerol,TG)、血清总胆固醇(total cholesterol,TC)、血尿素氮(blood urea nitrogen,BUN)、血肌酐(serum creatinine,Scr)、血尿酸(uric acid,UA)、谷丙转氨酶(glutamic pyruvic transaminase,ALT)、体质量指数(body mass index,BMI)及腰围。结果:对照组有效率为85.3%,研究组有效率为96.7%,两组患者临床疗效比较,差异具有统计学意义(P0.05)。两组患者治疗后FPG、2h PG、Hb A1c、低血糖发生率、TG、TC、BMI及腰围均低于治疗前,且研究组低于对照组,差异均有统计学意义(P0.05)。结论:中药肠内营养剂干预IGT,可改善不良指标,并且未见肝肾功能的损害,糖尿病发病率明显降低。

Abstract:Objective:To observe the clinical curative effect of traditional Chinese medicine enteral nutrition on impaired glucose tolerance(IGT).Methods:Three hundred patients with IGT were randomly divided into control group and study group,with 150 cases in each group.The ones in the control group were in accordance with TCM standard diagnose and treatment to reduce blood sugar and fat,loose weight,and do some exercise,while the ones in the study group were given traditional Chinese medicine enteral nutrition into specific staple food based on the treatment of the control group.The clinical efficacies of the two groups and the changes of TCM syndrome scores before and after treatment were compared.The two groups' FPG before and after the treatment,2 h blood sugar(2 hPG) after meals,glycosylated hemoglobin (HbA1c),hypoglycemia incidence,serum total cholesterol (TC),Three acyl glycerol(TG),Blood urea nitrogen (BUN),serum creatinine (Scr),uric acid (UA),Glutamic pyruvic transaminase (ALT),Body mass index(BMI) and waistline were observed.Results:The effective rate of the control group was 85.3%,while that of the study group was 96.7%,and the difference between the two groups was statistically significant (P < 0.05).After the treatment,FPG,2 hPG,HbA1 c,the incidence of hypoglycemia,TG,TC,BMI and waistline in the two groups were lower than those before the treatment,and the study group was lower than that of the control group,and all the differences were statistically signifi-cant(P < 0.05).Conclusion:Intervention of IGT with Chinese medicine enteral nutrition can improve the bad indexes,and there is no impairment of liver and kidney functions,and the incidence of diabetes is obviouslv lower.
