腹腔镜下全子宫切除术与剖腹全子宫切除术两种术式的临床对比分析

目的: 评估使用腹腔镜下全子宫切除术的安全性、可行性及优越性。方法: 120例需要择期行全子宫切除术的患者, ASAⅠ~Ⅱ, 年龄 31~58岁, 根据术式不同随机分为腹腔镜组 (A组n=60) 和剖腹组 (B组n=60)。对手术适应症、手术时间、术中出血量、术后恢复情况、住院时间及并发症发生情况进行对比分析。结果: 腹腔镜组平均手术时间较剖腹组稍长, 但无显著差异性 (P>0 .05)。术中出血量、术后并发症的发生率、术后肛门排气时间以及术后住院时间明显优于剖腹组,差异有显著性 (P<0. 01)。结论: 腹腔镜下全子宫切除术与剖腹全子宫切除术相比有损伤小、腹部切口小、术中和术后并发症的发生率低、恢复快、住院时间短等优点, 更具优越性。

Clinical comparative analysis of laparoscopically and abdominal incision total hysterectomy

Objective:To evaluate the safety, feasibility and advantage of laparoscopically total hysterectomy.Methods:120 cases with ASA Ⅰ~Ⅱ and indication of hysterectomy underwent total hysterectomy were randomized divided into two groups. The cases in Group A (n=60) were used by laparoscopy and those in group B (n=60) were used by abdominal incision. The adaptability of surgery, operative duration,the hemorrhage during operation,restitution after operation,hospitalization duration and the incidence of complications were compared.Results:The average time of operation was a litter longer in group A than that in group B (P>0.05),the hemorrhage during operation,restitution after operation,hospitalization duration and the complications were obviously more excellent in group A than those in group B (P<0.01).Conclusion:The advantages of laparoscopically total hysterectomy are less trauma,smaller abdominal incisions and recovering faster, which should be a preferable operation of hysterectomy.
