原发性高血压中医证型血管内皮功能的变化规律及其临床意义

目的探讨原发性高血压(EH)不同中医证型与血管内皮功能的变化规律及临床意义。方法1 2 0例EH患者按中医辨证分为肝阳上亢型( 3 0例)、阴虚阳亢型( 42例)、肝肾阴虚型( 2 6例)、阴阳两虚型( 2 2例)和正常血压对照组2 0例。分别检测内皮素(ET)、一氧化氮(NO)浓度并分析之间的变化。结果不同中医证型EH患者较正常对照组ET明显增高,NO明显降低。ET值:肝阳上亢型<阴虚阳亢型<肝肾阴虚型<阴阳两虚型。NO值:肝阳上亢型>阴虚阳亢型>肝肾阴虚型>阴阳两虚型。结论ET和NO可作为EH中医辨证分型和判断病情轻重程度的客观化指标。

The Changes and Their Clinical Meanings of Vascular Endothelial Function in Essential Hypertension with TCM Pattern Classification

Objective To investigate the changes and their clinical meanings of vascular endothelial function in essential hypertension with TCM pattern classification.Methods 120 patients with essential hypertension were divided into different patterns with TCM classification,i. e. liver yang excess (30 cases),yin deficiency and yang excess(42 cases),liver kidney yin deficiency (26 cases),yin and yang deficiency(22 cases) and a control group with normal blood pressure(20 cases).The concentration of plasma ET and serum NO were tested respectively and their changes were analyzed.Results In contrast with those of the control group,ET raised and NO declined obviously in all of the patterns with TCM classification, with the order of ET value from low to high as liver yang excess, yin deficiency and yang excess, liver kidney yin deficiency, yin and yang deficiency; and the order of NO value from high to low as liver yang excess, yin deficiency and yang excess,liver kidney yin deficiency, yin and yang deficieney.Conclusions ET and NO may be used as objective indices in analyzing the degrees and syndrome differentiations of essential hypertension.
