呼吸道非典型病原体检测技术的应用与评价

摘    要:目的:应用多种检测方法对临床常见呼吸道非典型病原体进行检测,并对检测结果进行分析和方法评价,为临床提供呼吸道感染性疾病病因诊断依据。方法:应用APAAP法、PCR法检测常见4种8型呼吸道病毒和MP病原,同时应用GICA法对流感病毒、快速培养法对MP进行检测,并分析比较检测结果。结果:441例样本中,病原体检出率为59.2%,其中混合感染率占阳性患者的36.8%。其中应用GICA法检测Flu A和Flu B与APAAP法总体一致率分别达90.0%和76.9%(Kappa值<0.75,一致性不理想),培养法检测MP与PCR法总体一致率达88.4%,(Kappa值<0.75,一致性不理想)。结论:非典型病原体是成人呼吸道感染的重要病原体,应用APAAP法、PCR法的方法能够为临床提供早期诊断和正确治疗的依据,但GICA法的检测敏感性和MP快速培养法的特异性有待于改进。

Abstract:Objective:To detect and analyze common respiratory atypical pathogens in clinical specimens through a variety of detection methods and further provide laboratory evidence for diagnosis of respiratory infection.Methods:Four classes(8 types) respiratory tract virus and MP were detected by APAAP and PCR.Flu was detected by GICA.MP was detected by rapid culture.Results:Detection rate of respiratory atypical pathogens was 59.2% in the 441 samples.The rate of mixed infection was 36.8%.The results of GICA detection of Flu have a low consistency of 90.0%(Flu A) and 76.9%(Flu B) with that of APAAP detection(Kappa<0.75).Rapid culture of MP has a low consistency of 88.4% with that of PCR detection(Kappa<0.75) in those samples.Conclusion:Respiratory tract virus and MP are important pathogens in respiratory tract infection.Using APAAP and PCR are able to provide evidence for early diagnosis and proper therapy.GICA and rapid culture require to be further improved.
