血清特异性抗原在前列腺良恶性肿瘤诊断中的应用

目的比较血清总前列腺特异性抗原（tPSA）、游离前列腺特异性抗原（fPSA）与tPSA的比值（f／t）、复合前列腺特异性抗原（cPSA）鉴别诊断前列腺增生（BPH）和前列腺癌（PCa）的价值。方法150例分为3组：健康对照组50例，前列腺增生组50例，前列腺癌50例，患者血清tPSA、fPSA浓度，计算f／t比值，cPsA／tPsA、fP—SA／cPSA比值。结果PCa组的cPSA／tPSA、fPSA／tPSA和fPSA／cPSA比值的均值与BPH组差异有统计学意义（P〈0．001），fPSA／cPSA与fPSA／tPSA比值密切相关（r=0．8810）。结论血清tPSA为前列腺的标志物，而f／t及fPsA／cPsA作为tPSA的辅助指标，在鉴别良恶性肿瘤中有重要意义。

Application of serum specific antigen in differential diagnosis of benign and malignant prostatic tumors

Objective To evaluate serum total prostate specific antigen （tPSA）, free prostate specific antigen （fPSA）/ tPSA ratio and complexed prostate specific antigen （cPSA） in the differential diagnosis of benign prostatic hyperplasia （BPH） and prostate cancer （PCa）. Methods Fifty healthy people （healthy control group）, 50 patients with BPH （BPH group） and 50 patients with PCa （PCa group） were in the study. Serum tPSA, fPSA and cPSA were measured by applying automated chemilluminescent immunoassay, cPSA/tPSA and fPSA/cPSA were calculated. Results The median cPSA/tPSA, fPSA/tPSA and fPSA/cPSA ratio were significantly different between BPH group and PCa group （P〈0. 001）. The fPSA/tPSA ratio was closely associated with the fPSA/cPSA ratio （r= 0. 881 0）. Conclusion Serum tPSA is a marker for diagnosis of prostatic carcinoma, and fPSA/tPSA and fPSA/cPSA as adjuvant markers to tPSA. They are of importance in the differential diagnosis of BPH and PCa.
