| 摘 要： | 目的 探讨肾巨大铸形结石的治疗方法。方法 采用肾窦内肾盂切开取石术治疗肾巨大铸形结石57例。结果 本组57例均取石成功，疗效满意。1例术后继发出血，需要再次手术止血；3例因残留结石下移至输尿管导致梗阻。形成尿漏，需作输尿管镜下气压弹道碎石，留置双J管；30例病人术后1-6个月内复查KUB；8例证实残留结石直径均小于9mm，再行ESWL。结论 肾窦内肾盂切开取石术是治疗肾巨大铸形结石的一种有效方法，安全、可靠、易掌握，对肾功能损害较小。

| Abstract: | Objective To study the treament of giant staghorn calculi.Methods The data of 57 cases treated with intrasinusal pyelolithotomy were analysised.Results Lithotomy was successful in all cases with average operation time 98 minutes,Blood transfusion during the procedure was required in 16 cases with an everage of 430ml, one case want to do second operation because bleeding, 3 cases want to do air-pressure lithotripsy and double J catheter placing for the treatment of ureteral obstruction due to residual calculus.postoperative examination of 30 cases with KUB revealed residual calculus in 8,the diameters being less than 9mm.Conclusions The procedure for giant staghorn calculi was easy, safe, and effective with renal parenchyma intact. |
