超声刀在行新辅助放疗后的中晚期食管癌手术中的应用

目的:观察超声刀在行新辅助放疗后的中晚期食管癌手术中的临床应用价值。方法:同期使用超声刀(23例)和电刀(18例)行食管癌切除术,对比观察两组的手术时间、术中出血量、术后24h胸腔引流量等指标。结果:所有病例均顺利完成手术,超声刀组平均手术时间(146.42±19.78)min,术中平均出血量(75.21±5.23)ml,术后24h平均胸腔引流量(204.33±24.24)ml,电刀组分别为(233.64±32.32)min,(147.85±16.21)ml,(325.45±35.25)ml,两组比较差异均有统计学意义(P均<0.05)。术后均无胸、腹腔内出血二次手术及手术相关并发症。结论:超声刀在行新辅助放疗后的中晚期食管癌手术中具有精确切割、损伤小、出血少、手术时间缩短、可以提高食管癌手术的安全性等优点,值得临床推广应用。

Application of Ultracision Harmonic Scalpel during Operation in Neo-adjuvant Radiotherapy Patients with Locally Advanced Esophageal Carcinoma

Objective： To investigate the clinic applied value of ultrasonic harmonic scalpel in neo-adjuvant radiotherapy patients with locally advanced esophageal carcinoma. Methods：The average operation time, hemorrhage and the quantity of thoracic drainage postoperatively in ultrasonic harmonic scalpel group（23 cases）and electrotome group （18 cases） were compared. Results.. All the operations were carried out successfully. In the ultrasonic harmonic scalpel group, the average operation time, hemorrhage and the quantity thoracic drainage flow were（146. 42±19. 78 ） rain, （ 75. 21±5. 23 ） ml, （204. 33±24. 24）ml,respectively,and those in electrotome group were（233. 64i32. 32）min, （147. 855：16.21）ml, and （325.45±35. 25）ml, respectively. There were significant difference between two groups in above indexes （P〈0.05）. No complications were found in both groups. Conclusion：Ultrasonic harmonic scalpel in neo-adjuvant radiotherapy patients with locally advanced esophageal carcinoma resection is worthy to be clinically popularized for its exact incision, little injury and hemorrhage, less operation time and more safety.
