死胎肝脏组织中HBV DNA表达的初步探讨

目的　观察死胎肝脏组织中HBVDNA表达的情况 ,探讨通过产妇传播到死胎肝脏组织中的HBV是否存在复制。方法　采集 40例乙型肝炎产妇产下的死胎 ,常规尸检 ,取肝脏组织。采用原位分子杂交法 ,检测其中的HBVDNA ,回访产妇产前静脉血HBV的检测结果。结果　死胎肝脏组织中HBVDNA的检出率为 87 5 % (3 5 /4 0 )。产妇产前静脉血HBV呈高、低和无复制状态时 ,肝脏组织中HBVDNA的检出率分别为 75 0 % (3 /4 )、85 0 % (17/2 0 )和 94 0 % (15 /16) ;高、低、无复制者之间相互比较 ,差异不显著 (P >0 0 5 )。HBVDNA颗粒分布在肝细胞浆内、毛细胆管上皮和管腔内 ,少数分布在肝细胞核上。结论　死胎肝脏组织中有HBV复制 ,死胎肝脏组织中的HBVDNA表达与产妇血清中HBV复制状态无关。

The primary study on the expression of HBV DNA in the liver tissues of died fetus

Aim To observe the expression of HBV DNA in died fetus and to discuss whether there were copies of HBV by puerperal delivery in the liver tissues of the fetus.Methods 40 cases of died fetus were made autopsy for liver tissues and their HBV DNA was detected by in situ molecule hybridization technique, then retrospected the detection of HBV in the blood of parturient before delivery.Results For the liver tissues, the positive rate was 87.5%(35/40) for HBV DNA detection.When HBV showed high, low and without replication status, it was 87.5%,85.0% and 94.0% respectively in the liver tissues, there were no significant differences among the high, low and without replication status ( P >0.05).HBV DNA was mainly distributed in the liver cytoplasm, lumens and epithelium of bile capillaries, and a little was also distributed in the nuclei.Conclusions There is HBV replication in the liver tissues of the died fetus. There is no relation between the expression of HBV DNA in the died fetus and the replication status in the sera of parturient.
