2型糖尿病患者尿单核细胞趋化蛋白-1排泄的变化及其意义

摘    要:目的观察2型糖尿病(type 2 diabetes mellitus,T2DM)患者尿单核细胞趋化蛋白-1(MCP-1)排泄的变化并分析尿MCP一1排泄与尿白蛋白排泄的关系。方法正常对照组20例(A组),56例2型糖尿病患者据其尿白蛋白/肌酐比值(the ratio of urinary albumin to creatinine ACR:mg·g^-1·Cr^-1)分为正常白蛋白尿组(B组,N=21),微量白蛋白尿组(C组,N=20),临床白蛋白尿组(D组,N=17)。检测所有人选者尿MCP-1/尿肌酐(Ucr)比值及其相关因素。结果与对照组比较,B组、C组、D组在年龄、性别、体重指数方面差异无统计学意义;与A组比较,B组尿MCP-1/Ucr水平显著升高(P〈0.01),C组和D组尿MCP-1/Ucr水平进一步升高(P〈0.01);尿MCP-1/Ucr水平与UAIb/Ucr、HbAlc、血清TC和TG呈正相关。结论2型糖尿病患者尿MCP-1排泄增加,并与糖尿病。肾病的病情程度有关。

Abstract:Objective To observe the changes of the urinary MCP-1 excretion, and analyse its relationship with albuminuria in T2DM. Methods 20 normal controls (group A, N = 20) and 56 diabetics were enrolled in this study, with diabetics, were divided into 3 groups according to (ACR: mg·g^-1· Cr^-1 ) : normoalbnminuria group (group B, N = 21 ), micro-albuminuria group (group C, N = 20), and macroalbuminuria group (group D, N = 17). Urinary MCP-1 were measured by enzyme-linked immunosorbent assay (ELISA). Results There were no significant differences between normal control group and group B, C and D in age, sex, and body mass index; Compared with group A, the UMCP-1/ urinary creatinine (Ucr) in group B increased significantly (P〈0.01 ), and increased further in group C and group D (P 〈 0.01 ); UMCP-1/Ucr was positively related with ACR, HbAlc, serum TC and TG. Conclusions UMCP-1 excretion is increased in type 2 diabetics, which is related to the degree of dia- betic nephropathy.
