系统性红斑狼疮患者血清基质金属蛋白酶-9检测及其临床意义

目的研究系统性红斑狼疮(SLE)患者血清基质金属蛋白酶-9(MMP-9)水平,并探讨其临床意义。方法采用双抗体夹心酶联免疫吸附测定法检测血清MMP-9水平。比较分析28例健康人与67例SLE患者,疾病活动组(49例)与稳定组(18例),以及16例活动组患者经治疗后病情稳定3个月后与治疗前血清MMP-9情况。结果SLE患者血清MMP-9水平明显低于正常对照组(<0.01);活动组与稳定组差异无显著意义(>0.05),而且血清MMP-9水平与各临床表现均无相关。血清MMP-9水平在抗SM抗体阳性组明显低于阴性组(<0.01),血清IgM升高组明显低于血清IgM降低组(<0.01),血清IgG组、血清C3组间差异无显著意义(>0.05)。16例活动组患者经治疗后病情稳定,3个月后血清MMP-9水平与治疗前无明显变化(>0.05)。结论SLE患者血清MMP-9水平明显低于正常对照组,但血清MMP-9水平不能反映出疾病的活动状态,故而不能作为SLE疾病活动和疗效评价良好指标。

Detection of serum matrix metalloproteinase-9 in patients with systemic lupus erythematosus and its clinical significance

Objective Todetect the level of serum matrix metalloproteinase-9 (MMP-9) in patients with systemic lupus erythematosus (SLE) and investigate its clinical significance.Methods Double antibody enzyme-linked immunosorbent assay was applied to detect serum MMP-9 level. Serum MMP-9 level in 28 healthy persons was compared with that in the 67 SLE patients ( 49 patients at active phase, 18 patients at stable stage), and serum MMP-9 level was compared in 16 patients at active stage before and after the treatmen.Results Serum MMP-9 level in SLE patients was significantly lower that in the normal controls (P<0.01); there was no significant difference between the active and stable groups (P>0.05), and the serum MMP-9 level was not relevant to clinical manifestations. Serum MMP-9 level in the group with positve anti-Sm antiboy was distinctly lower than that in the group with negative anti-Sm antibody (P<0.01), and serum MMP-9 level in the group with increased level of IgM was markedly lower than that in the group with decreased level of IgM(P<0.01). There was no significant difference between serum IgG group and serum C3 group (P>0.05). As for 16 patients in the active group, there was no significant difference in serum MMP-9 level between 3 months after treatment and before treatment.Conclusion Serum MMP-9 level in SLE patients is markedly lower than that in the normal controls, but serum MMP-9 level can not display active state of SLE. It is not a good index for active state and therapeutic effect of SLE.
