摘 要: | 目的探讨LDL-C+ABCD2评分法对短暂性脑缺血发作后7 d内发生脑梗死的预测价值。方法用ABCD2评分法和LDL-C+ABCD2评分法分别测定106例TIA患者的评分,并观察TIA后7d内脑梗死的发生率。结果LDL-C+ABCD2评分法和ABCD2评分法的曲线下面积(95%CI)分别为0.849(0.771-0.927)和0.844(0.768-0.920)。采用LDL-C+ABCD2评分中TIA患者中低、中和高危组TIA后7 d内发生脑梗死的比例分别为2.1%、33.3%和64.3%,不同脑卒中风险分层的脑梗死发生率有显著差异(P<0.05)。结论ABCD2评分法是目前临床预测TIA短期进展为脑梗死的有效的方法 ,结合LDL-C检查结果能提高预测的准确性。

Abstract: | Objective To explore the ability of LDL-C+ ABCD2 score in predicting cerebral infarction of transient ischemic attack(TIA) at 7th day.Methods 106 TIA patients were evaluated according to ABCD2 criteria and LDL-C+ABCD2 criteria.The occurrence of cerebral infarction was observed in 7 days.Results The AUCs(95%CI) of LDL-C+ ABCD2 and ABCD2 were 0.849(0.771-0.927) and 0.844(0.768-0.920).With an LDL-C+ABCD2 score,the 7-day risk of stroke in patients of low risk、moderate risk and high risk was 2.1%、33.3% and 64.3%.The incidence of cerebral infarction of TIA patients with different scores had statistically significant difference(P<0.05).Compared with non-infarction group,the proportions of infarction-patients with LDL-C ≥ 3.10mmol/L are independent risk factors(P<0.05).Conclusion The ABCD2 criteria is effective for predicting the occurrence of cerebral infarction after short time of TIA.and an enhanced accuracy may be expected by LDL-C+ ABCD2 score. |
