右美托咪啶静脉注射辅助全身麻醉效果及安全性分析

目的分析右美托咪啶静脉注射辅助全身麻醉的效果及安全性。方法 选择我院2009年1月～2010年12月在行开腹手术时采用全身麻醉的患者80例。随机分为观察组和对照组。观察组应用右美托咪啶辅助全麻,对照组注射等量生理盐水辅助全麻。记录两组的Ramsay镇静评分、SpO2、血压、心率、呼吸频率、异丙酚、芬太尼、异氟烷用量、麻醉时间、手术时间、拔管时间、清醒时间、不良反应发生率。结果两组患者给药前Ramsay镇静评分无差异（P〉0.05）。给药后5min、10min时,观察组的分数均高于对照组（P〈0.05）,观察组应用异丙酚、芬太尼及异氟烷的用量均少于对照组（P〈0.05）,但两组的麻醉、手术、拔管及清醒时间、不良反应发生率差异无统计学意义（P〉0.05）。结论右美托咪啶是一种疗效安全,镇静作用较强的新型药物,适合应用于辅助全身麻醉,值得推广。

Efficiency and safety of intravenous dexmedetomidine as an adjunct to general anesthesia

Objective To analyse the efficacy and safety of intravenous dexmedetomidine as an adjunct to general anesthesia. Methods 80 patients from Jan 2009 to Dec 2010 randomly assigned to control group and observation group.The patients in observation group received dexmedetomidine injected and that in control group received equal volume of normal saline.BP,HR,respiratory rate（RR）,SpO2 emergence time,extubation time and level of satisfaction with quality of anesthesia and Ramsay score was measured and recorded.The quality of anesthesia was scored.The changes in postoperative renal and hepatic function and the adverse reactions were recorded. Results The average Ramsay scores obtained were better than control group. The average dose of propofol required for induction of anesthesia was significantly less in observation group than in control group.There were no significant differences in emergence time,extubation time and level of satisfaction with quality of anesthesia between the 2 groups.There were no significant differences in the changes in postoperative renal and hepatic function and the incidence of adverse reactions between the 2 groups. Conclusion Dexmedetomidine has sedative effect and can reduce the amount of propofol,fentanyl and isoflurane,it is safe to be used as an adjunct to general anesthesia.
