颈动脉彩超联合TCD评估在糖尿病并发缺血性脑血管疾病早期检测中的应用

目的：探讨彩色多普勒超声（CDFI）经颅多普勒超声（TCD）对2型糖尿病并发缺血性脑血管病患者颈动脉及颅内动脉血流动力学早期改变的声像特点及临床应用价值。方法：根据病情及临床症状的不同把300例2型糖尿病患者分成3组。3组患者均行颈动脉CDFI及颅内动脉TCD检查。使用CDFI测量颈动脉内膜-中层厚度（IMT）,观察管腔是否有斑块、斑块性质、管腔是否狭窄闭塞、狭窄的程度及血流动力学变化。使用TCD检查颅内动脉血流参数并记录。结果：CDFI检出研究组二患者IMT增厚、粥样斑块的检出率明显高于研究组一患者,而研究组三患者的IMT增厚、粥样斑块的检出率明显高于研究组一和研究组二,其程度重于前两组。结论：CDFI和TCD联合检查发现糖尿病并发缺血性脑血管病患者早期会出现颅内动脉的流速、搏动指数、阻力指数的改变和颈动脉IMT增厚,这都是缺血性脑卒中危险性的早期指征,具有重要的临床价值。

Objective： To explore the sonographic features and the clinical value of color doppler flow imaging（CDFI）, Transcranial doppler （TCD） in detecting the changes at the early stages of Carotid Artery and Cerebral Arteries in the patients with crerbrovascular disease in type 2 diabetes mellitus. Methods： Basis of clinical symptoms of illness, 300 cases of the Ⅱdiabetes patients were divided into three groups.CDFI and TCD were performed in three groups patients.CDFI was used to find the changes of the blood hemodynamics and TCD was used to find the ultrasonographic features. Results： CDFI showed： the abnormality rate of 3 studied group was the highest, The second was the 2 studied group, the third was 1 studied group. Conclusion： CDFI and TCD were used together to find the changes of the blood hemodynamics and the ultrasonographic features Cerebral Arteries. They are primitive draft of the chemic stroke . They are great value in clinical.
