BCL-2,p53蛋白在乳腺原位癌中的表达

摘    要:目的 :探讨BCL - 2 ,p53蛋白在乳腺原位癌及正常乳腺组织中的表达及相互关系。方法 :采用免疫组化检测 15例正常乳腺组织 ,10例小叶原位癌及 4 5例导管原位癌 (DuctalCarcinomainsitu ,DCIS)。结果 :BCL - 2蛋白在正常乳腺组织及小叶原位癌中的表达均阳性 ,p53蛋白均阴性 ,基因表型呈BCL - 2 /p53-。4 5例DCIS中 ,BCL - 2蛋白阳性率 77.8% (35/ 4 5) ,p53蛋白阳性率 2 4 .2 % (11/ 4 5)。BCL - 2蛋白表达与DCIS组织学分级密切相关 (P <0 .0 1)。DCIS中有 6 6 .7%呈BCL - 2 /p53-表型 ,多为高分化和中分化病例 ,还有 33.3% ,多为差分化病例 ,坏死出现率比呈BCL - 2 /p53-表型的病例高 (P <0 .0 1)。结论 :正常乳腺组织及小叶原位癌均呈BCL - 2 /p53-表型 ;6 6 .7%导管原位癌与正常乳腺组织具有相似的凋亡调控基因 (BCL - 2 /p53- ) ,非正常表型DCLS病例多代表分化差的病例 ,可能这些病例不受正常凋亡调控基因的控制 ,结果导致肿瘤细胞的存活和生长。

Abstract:Objective: To probe into the relationship and expression of BCL-2 and p53 protein in breast carcinoma in situ.Methods:By means of immunohistochemistry,BCL-2 and p53 protein expressions were determined in 15 cases of normal mammary gland,10 cases of lobular carcinoma in situ and 45 cases of ductal carcinoma in situ.Results:The expression of BCL-2 in all normal breast tissue and lobular carcinoma in situ are positive,p53 protein negative,and genotype appears BCL-2 /p53.The positive rate of BCL-2 in 45 DCLS patients was 77.8%(35/45),and that of p53 protein 24.2%(11/45).There exists a close correlation between BCL-2 protein expression and the histologic grade of DCLS(P<0.01).The DCL-2 /p53- phenotype was observed in 66.7% of the the DCIS cases,and most lesions represented examples of well-differentiated and intermediatel differentiated cases.In contrast,the remaining 33.3% of DCIS cases:belong to poorly differentiated DCIS.Necroses were significantly more common in DCIS cases with altered BCL-2 and p53 expression than in those with BCL-2 /p53- phenotype.Conclusions: The BCL-2 /p53- phenotype was observed in normal breast tissue and breast carcinoma in situ.Most DCIS lesions are BCL-2 /p53-,similar to normal epithelium.However, a minority of DCIS lession show the combination of BCL-2 and p53 expression that differ from normal breast epithelium,represented examples of poorly differentiated cases.It is possible that such lessions may represent the regulation of dying_controlled genes is no longer under normal control mechanism, resulting in enchances tumor cell survival and tumor growth
