右旋美托咪定应用于颈动脉支架置入术中的临床观察

目的：对比观察右旋美托咪定用于颈动脉支架置入术中的镇静效果及可行性.方法：40例拟行颈动脉支架置入术的患者,随机分为右旋美托咪定组（D组）与咪达唑仑＋舒芬太尼组（C组）,每组20例,采用静脉注射麻醉,记录给药即刻（T0）、给药后5min（T1）、10min（T2）、15min（T3）、30min （T4）及术毕（T5）时点的MAP、HR、RR、SpO：及Ramsay评分.结果：与基础值比较,D组T2-T4时点MAP和HR降低,T2、T3时点Ramsay评分升高；C组T2时点MAP降低,T2、T3时点Ramsay评分升高.D组与C组比较,在T3、T4时点MAP、HR降低,,T3时点Ramsay评分升高.术中呼吸频率,术后24h恶心呕吐发生率差异无统计学意义.结论：右旋美托咪定具有良好的镇静效果和安全性,是行颈动脉支架置入术麻醉较理想的药物选择.

The sedative effect of dexmedetomidine during carotid angioplasty and stenting

Objective： To evaluate the efficacy of dexmedetomidine application during carotid angioplas- ty and stenting. Methods ： Forty patients scheduled for carotid angioplasty and stenting were randomly into group dexedetomidine（ group D） and midazolam plus sufentany group （ group C ）. MAP, HR, RR, SpO/ and Ramsay score were recorded before administration（T0） and,5 min（Yl） ,10 min（T2） ,15 min（T3） ,30 min（T4） and the end of operation（ T5 ） after sedative administration. Results：Campared with the baseline, MAP and HR de- creased at T3-T5 and Ramsay scores increased at T3-T4 in group D. MAP decreased at T3 and Ramsay scores increased at T3-T4 in group C. Compared with group C, MAP and HR decreased at T4-T5 and Ramsay scores increased at T4 in group D. No differences between groups were noted for postoperative nausea and vomiting and Ramsay score. Conclusion：Dexmedetomidine is an ideal sedative drugs can be safely used in carotid angioplas- ty and stenting surgery sedation.
