膀胱移行细胞癌血管内皮生长因子表达与预后的关系

目的探讨与膀胱移行细胞癌预后相关的新检测指标。方法采用免疫组化S—P法,检侧68例原发性膀胱移行细胞癌和10例癌旁正常膀胱组织标本血管内皮生长因子(VEGF)的表达,结合临床资料,探讨其相互联系,以及其表达与患者预后的关系。结果68例膀胱移行细胞癌组织中VEGF阳性表达率为45．6％(31／68),与病理分级、临床分期、血管浸润有关(P<0．01),有无淋巴结转移间有显著性差异(P< 0．001):10例癌旁正常膀胱黏膜VEGF表达均为阴性;VEGF表达阳性者生存超过5年者(39．3％)明显低于VEGF表达阴性者(93．7％,P<0．05)。结论VEGF表达与膀胱移行细胞癌的恶性程度密切相关,检测VEGF表达对判断膀胱移行细胞癌预后有重要意义。

Expression of VEGF in transitional cell of bladder and its relationship with the prognosis

Objective To invesligale a new diagnostic index for the prognosis of bladder transitional cell cancer.Methods Expression of vascular endolhelial cell growth factor (VEGF)in 68 cases of primary transitional cell carcinoma(TCC)and 10 cases of normal bladder tissue surrouding neoplasms were assayed using immunohistochemical technique (S-P method),and their relation to the pathological grading, clinical stages and prognosis were analyzed.Results All normal bladder tissues showed negative staining while the rate of expression of VEGF in neoplastic tissues was 45.6%(31/68).The positive rale of VEGF expression in poorly differentiated and invasive bladder tumors were higher than those of well differentiated and superficial tumors(P<0.01).The expression of VEGF in carcinoma tissue showed positive relationship with tumor vascular invasion (P<0.01),and have relationship with lymph node metastasis (P< 0.001). The 5-year survival time(39.3%) of patients with expression of VEGF was significantly lower than that (93.7%) of patients without expression of VEGF (P<0.05).Conclusion Expression of VEGF has important influence on the biological behavior of bladder carcinoma,and expression of VEGF may aid predicting the prognosis of bladder transitional cell cancer.
