洛赛克对梗阻性黄疸围手术期胃内pH的影响及其临床意义

目的探讨洛赛克在梗阻性黄疸围手术期应用对胃内pH的影响及其预防急性胃粘膜病变的意义。方法将40例患者分成3组,术前均肌注阿托品0.5mg,A组术前1h静注洛赛克40mg,B组术后1h静注洛赛克40mg,C组术后未用任何影响胃酸分泌的药物。动态监测胃内pH值变化,术后记录相关上消化道出血症状。结果术中胃内pH增高,与术前相比,A组P<0.01,B、C组P<0.05,A组术中、术后保持pH值6上下,B、C组术后pH值明显下降,术后1h与术前比较P<0.05。术后出现消化道出血症状者,A组1例,B组3例,C组5例。结论洛赛克对梗阻性黄疸急性胃粘膜病变有预防作用,洛赛克40mg术前静注效果最佳。

Effect and clinical significance of losec on the intragastric pH in patients receiving obstructive jaundice surgery

Objective The aim of the study was to investigate the effect of losec on the intragastric pH and the prevention of acute ulcer hemorrhage in the patients of obstructive jaundice around the operation.Methods Forty patients were divided into three groups at random and all received atopine 0.5mg intramuscularly before operation.A group received losec 40mg intravenously at 1 hour before the operation. B group received losec 40mg intravenously at 1 hour after the operation.C group didn't receive any drugs that affected the intragastric pH. 24-hour intragastric pH moitoring was carried out and the results of peptic ulcer hemorrhage were recorded.Results The intragastic pH increased during the operation,compared with the results before the operation,A group P<0.01,B,C group P<0.05.The intragastric pH in A group was about 6 during and after the operation.The results decreased in B, C group after the operation,and compared with that before the operation,P<0.05.Conclusion Losec was useful in the prevention of acute ulcer hemorrhage,in the patients of obstructive jaundice aroud the operation. And to receive losec 40mg intravenously before the operation attained the best clinical efficacy.
