~(99m)Tc-MIBI显像对乳腺肿块鉴别诊断的价值

摘    要:目的 评价99mTc MIBI乳腺肿块显像的规律及特征以及诊断乳腺癌原发病灶的价值。方法  2 8例患者 ,因乳腺可触及肿块而进行99mTc MIBI显像检查。另外有 14例乳腺未触及异常肿块的患者作为对照组。由肘静脉注射99mTc MIBI 1110MBq ,分别于注射后 5 ,30和 6 0min进行显像 ,于左侧位、右侧位及前后位观察 ,视野包括双侧乳腺及双侧腋窝 ,所有患者在显像 1周内行手术治疗 ,并行活组织病理学检查。结果 99mTc MIBI乳腺显像 ,原发病灶探查的敏感性是 89% ,特异性 90 %。可检查到的最小乳腺肿块为 1.2cm× 1.5cm× 0 .8cm ,病理检查为腺癌 ,乳腺肿块的显像与组织病理学结构有密切的联系。结论 99mTc MIBI乳腺显像在鉴别良恶性肿瘤方面是有效的检查手段

Abstract:Objective This study was to evaluate the value of 99m Tc MIBI imaging in the detection of primery brest cancinomas and to assess imaging characteristics of the lessions Methods 28 patients underwent 99m Tc MIBI scintigraphy of the breast because of palpable breast masses An additional 14 femals with no palpable breast abnormalities were also studied as controls The early and delay images was acquined at 5 min, 30 min and 60 min after injection of 1 110 MBq 99m Tc MIBI intravenously Three planar views, right and left latered prone and anterior supine thoracie views, were obtained The entire breast and ipsilateral axillary region were included in the field of view Excisional breast biopsy were performed in all patients within 1 week after seintimammography Results The sensitivity of scintimammagraphy for detecting primary breast cancer was 89% And the speciflcity was 90% The smallest detectable carcinoma was an adenocarcinoma measuring 1.2 cm×1.5 cm×0.8 cm Meanwhile the histopathologic features of the lesions greatly influenced their imaging characteristics Conclusion 99m Tc MIBI scintigraphy of breast lesions is an effective means of differentiating benign from malignant lesions
