91例促排卵成功案例临床分析

目的:分析不同促排卵方法对排卵障碍性不孕症治疗的效果、并发症的防治及缺陷。方法:将采用不同的促排卵方法治疗成功的91例排卵障碍性不孕症患者分为4组:克罗米芬(CC)组、人绝经期促性腺激素(HMG)+血清人绒毛膜促性腺激素(HCG)组、芳香化酶抑制剂(来曲唑)组和夏桂成名老中医的补肾促排卵汤(中药)组,比较其妊娠率、多胎率、流产率及卵巢过度刺激综合征(OHSS)发生率及不良反应。结果:CC组、HMG+HCG组、来曲唑组、中药组四组的妊娠率分别为15.38%、20.408%、24.14%、9.09%,前三组组间的差异均无统计学意义(P>0.05),前三组与中药组的差异均有统计学意义(P<0.05);前两组与后两组间的流产率、多胎率及OHSS发生率的差异有统计学意义(P<0.05)。结论:各组在促排卵过程中,各有各的优势及弊端,故仍需大量的临床试验资料来进一步评估各种促排卵药物的利弊,以便找到一种更安全、有效的促排卵药物,更好地用于指导临床用药。

Successful ovulation for 91 patients with anovulatory infertility

Objectives: To analyze the effects,complication prevention and disadvantages of different methods of ovulation promotion in anovulatory infertility patients.Method: Ninety-one patients with anovulatory infertility received successful ovulation treatment and were divided into four groups according to their different treatment methods: Clomiphene citrate(CC) group,Human menopausal gonadotropin(HMG) +Human chorionic gonadotropin(HCG) group,Aromatase inhibitor(letrozole) group and Mr xia’s TCM treatment group: decoction for invigorating kidney and promoting ovulation(fired angelica,red peony root,radix rehmanniae,yam,paeonol,tuckahoe each 10g,cornus officinalis,teasel,dodder,antler piece first decoct the 10g,safflower 5g) group.The four groups were compared in terms of pregnancy rate,multiple pregnancy rate,miscarriage rate,and ovarian hyperstimulation syndrome(OHSS) rate and adverse reaction rate.Results: The pregnancy rates in CC group,HMG+HCG group,letrozole group,Chinese medicine group were 15.38%、20.408%、24.14%、9.09% respectively.The first three groups showed no statistical difference(P>0.05),while the Chinese medicine group showed significant difference from all the other three groups(P<0.05).The abortion rate,multiple pregnancy rate and OHSS rate showed significant difference between the first two groups and the last two groups(P<0.05).Conclusion: In ovulation promotion process,all the four groups indicated their advantage and disadvantage.A large amount of clinical data is needed to evaluate the effects of these ovulation promotion drugs so as to find effective and safe treatment for anovulatory infertility patients.
