直肠癌术后吻合口瘘发生原因及防治分析

目的探讨直肠癌术后吻合口瘘发生的原因及防治策略。方法回顾性分析2002年9月—2013年11月间我院手术治疗285例直肠癌患者的临床资料。结果 285例直肠癌患者术后发生吻合口瘘共19例,发生率为6.67%,采取非手术治疗者16例,行横结肠造瘘手术治疗者3例,均成功治愈,瘘口平均愈合时间为（29.2±5.6）d;年龄、肿瘤位置、合并糖尿病、身体状况均对吻合口瘘的发生有明显影响（P〈0.05）。结论年龄、肿瘤位置、合并糖尿病、患者身体状况等是吻合口愈合的重要影响因素,提高术中吻合质量、及早发现、合理治疗是减少吻合口瘘发生及提高患者预后的关键。

Reason and prevention of postoperation stomal leak of rectal cancer

Objective To investigate the mason and prevention of postoperation stomal leak of rectal cancer. Methods To retrospectively analyze the clinical data of 285 rectal cancer patients receive operation from January 2002 to December 2013 in our hospital. Results There were 19 （6.67%） stomal leak cases among the 285 rectal cancer patients, non-surgical treatment in 16 patients and surgery in 3 patients. All patients were successfully close the fistula, the average healing time was （29.2 ± 5.6）d. The age,tumor location, diabetes mellitus, and condition of patients were significant effect to the incidence of stomal leak after surgical （P〈0.05）.Conclusion The age, tumor location,diabetes meUitus,and condition of patients are important influence factors for the stomal healing. Improve the stapling technique, early diagnosis and reasonable treatment can reduce incidence rate of stomal leak and improve prognosis of patients.
