南昌市高中生童年期不良经历与自杀相关行为的研究

摘    要:目的 了解高中生自杀意念、自杀计划现状及童年期不良经历,探索童年期不良经历与自杀意念、自杀计划的关系,掌握童年期不良经历对自杀意念、自杀计划的累积效应。方法 2015年5月,采取分层整群随机抽样方法,将南昌市城区34所高中按中考录取分数线由高到低排序随机抽取5个学校,共调查高中生884名。结果 884名高中生中125人(14.14%)有自杀意念,55人(6.22%)有自杀计划。logistic分析表明,躯体虐待(OR=3.002,P<0.001),家庭成员有精神异常者(OR=3.465,P<0.001)与自杀意念呈正性相关;性虐待(OR=3.105,P=0.008)、躯体虐待(OR=3.439,P<0.001),家庭成员有精神异常者(OR=5.295,P<0.001)与自杀计划呈正性相关。累积效应分析发现,随着童年期不良经历数目的增加,自杀意念、自杀计划发生的风险增加,有三种及以上不良经历者产生自杀意念OR=10.836,产生自杀计划OR=22.660。结论 童年期不良经历是高中生自杀意念、自杀计划发生的危险因素,加强对有童年期不良经历青少年的干预可有效防控高中生自杀相关行为。

Abstract:Objective To understand the incidence of suicidal ideation, suicidal planning and adverse childhood experiences in high school students, and to explore the relationship among adverse childhood experiences and suicidal ideation and suicidal planning, so as to master the cumulative effect of adverse childhood experiences on suicidal ideation and suicidal planning. Methods A stratified cluster random sampling method was adopted to select 5 schools from 34 high schools in Nanchang City according to the admission score of the middle school entrance examination from high to low. A total of 884 senior high school students were surveyed. Results Among the 884 senior high school students, 125(14.14%) had suicidal ideation, and 55(6.22%) had suicidal plan. Physical abuse(OR=3.002, P<0.001) and mental abnormality in family members(OR=3.465, P<0.001) were positively correlated with suicidal ideation. Sexual abuse(OR=3.105, P=0.008), physical abuse(OR=3.439, P<0.001) and mental abnormality of family members(OR=5.295, P<0.001) were positively correlated with suicide planning through Logistic analysis. With the increase of the number of adverse childhood experiences, the risk of suicidal ideation and suicide plan increased. People with three or more adverse childhood experiences had suicidal ideation OR=10.836 and suicide planning OR=22.660 through cumulative effect analysis. Conclusion Adverse childhood experience is an important risk factor for suicide ideation and suicide planning in senior high school students. Strengthening the intervention of adolescents with adverse childhood experiences can effectively prevent and control suicidal behaviors of senior high school students.
