亚低温脑保护在创伤后继发性脑损害治疗中的应用

摘    要:目的 探索亚低温脑保护在颅脑损伤治疗中应用的可行性及疗效。方法  85例颅脑损伤患者分为a .亚低温组 :3 1例 ,于伤后 2 4小时内接受亚低温治疗 ,直肠温度控制在 3 3℃~ 3 5℃ ,维持 3~ 7天。b .常规治疗组 :5 4例 ,于伤后 2 4小时内常规治疗。根据COS预后评估系统评估两组患者疗效。结果 经治疗后 ,亚低温组良好 16例 ,中残 6例 ,重残 4例 ,死亡 5例 ,恢复良好率为 5 1.61%。与常规组比较 ,差异有显著性意义 (P <0 .0 5 )。结论 亚低温具有减轻颅脑损伤后的脑水肿 ,降低颅内压 ,防止神经细胞凋亡的作用 ,伤后 2 4小时内接受亚低温治疗 ,疗效确切 ,可降低病死率 ,提高生存质量

Abstract:Objective To study the feasibility and effect of mild hypothermia for brain protection in the treatment of secondary braininjuries. Methods A total of 85 patients with brain injuries were divided into 2 groups, mild hypothermia therapeutic group (MHTG,31 Cases) and conventional therapeutic group (CTG, 54 Cases). In MHTC, mild hypothermiawas performed in 24 hours after injury, with the rectal temperature of the patients kept at 33℃~35℃ for 3~7 days. In CTG, conventional treatment were administered in 24 hours after injury. The therapeutic effects of the 2 groups were evaluated by COS.Results After treatment, there were 16 cases of good recovery, 6 cases of moderate disablement, 4 cases of severe disablement, and 5 cases of death; The good recovery rate was 51.61% in MHTG, which was higher than that in CTG ( P <0.05) Conclusion Mild hypothermia is a effeitive mean in decreasing cerebral edema and ICPand protecting from nerve cell apoptosls. The mild hypothermia should be carried out within 24 hours after brain injury. It can reduce the mortality and improve the prognosis.
