EV71感染手足口病患儿免疫功能临床分析

目的:研究EV71感染手足口病不同程度患儿T细胞亚群、免疫球蛋白水平变化,探讨患儿免疫功能及为临床治疗提供依据。方法:用流式细胞仪检测EV71感染手足口病患儿急性期和正常儿童外周血T细胞亚群,用免疫比浊法检测血清IgA、IgG和IgM水平。结果:EV71感染手足口病重度组T细胞亚群CD3+、CD4+、CD8+均低于普通组和对照组(P<0.01);普通组T细胞亚群CD3+、CD4+分别低于对照组(P<0.01),而CD8+T细胞在普通组和对照组差异无统计学意义(P>0.05);CD4+/CD8+在重度组中低于对照组,其余各组间差异无统计学意义(P>0.05)。普通组和重度组IgG和IgM水平均高于对照组(P<0.01),而IgG和IgM水平在普通组和重度组差异无统计学意义(P>0.05);各组间IgA水平差异均无统计学意义(P>0.05)。结论:EV71感染手足口病患儿细胞免疫功能降低,体液免疫反应增强,病情发展可能与患儿免疫状态紊乱有关。

Clinical significance of immune function of the children with EV71 infection

Objective:To evaluate the clinical significance of the level of T-lymphocyte subsets and Immunoglobulin of children with EV71 infection.Methods:Peripheral blood samples were collected from the children with EV71 infection and the healthy children,then T-lymphocyte subsets were detected by flow cytometry,and the IgA,IgG and IgM were tested by the immunoturbidimetric assay.Results:The percentages of T-lymphocyte subsets CD3+,CD4+ and CD8+ in EV71 infection severe group were lower than those in control and mild group(P<0.01),the percentages of CD3+ and CD4+ in mild group were lower than those in control group(P<0.01),but the percentages of CD8+ were not significantly different between the control and mild group(P>0.05).The ratio of CD4+/CD8+ in severe group were lower than that in control group,while those were not significantly different between the others groups(P>0.05).The concentrations of IgG and IgM in severe and mild group were higher than those in control group(P<0.01),while those were not significantly different between the severe and mild group(P>0.05).The concentrations of IgA were not significantly different between those groups(P>0.05).Conclusion:The cellular immunity function of the children physical with EV71 infection are decreased,and humoural immunoreactions are increased,the severe infection may relate to the immunity state imbalance.
