手术治疗股骨远端C型骨折疗效观察

目的总结手术治疗股骨远端C型骨折的疗效及预后：方法回顾性分析手术治疗的39例股骨远端C型骨折病历，其中使用髁支持钢板20例，髁动力螺钉13例，股骨髁上交锁髓内钉6例（7个肢体）。对伴有前交叉韧带、膝侧副韧带损伤者同期重建或修复。术后早期膝关节功能锻炼。结果34例获6～24个月随访，33例骨性愈合，愈合时间5～13个月，1例骨不连，3例发生膝内翻，2例膝外翻畸形。疗效评定：优16例，良8例，可6例，差4例。结论精确的解剖复位、合适的内固定、同期修复损伤韧带及术后早期功能锻炼是股骨远端C型骨折治疗的关键。

Surgical treatment of C-type fractures of the distal femur

Objective To summarize the effect and prognosis of surgical treatment of C-type fractures of the distal femur.Methods A retrospective study was done for the 39 patients with C-type fractures of the distal femur who had received operative treatment from 1999 to 2004 in our center.Condylar buttress plate,dynamic condylar screw and supracondylar nail were used in 20,13 and 6 patients(7 limbs) respectively.Results The 6-24 months' follow-up showed 33 bone union,1 nonunion,3 entad varus angulation and 2 outward varus angulation.The treatment outcome was evaluated as excellent in16 patients,good in 8,fair in 6 and poor in 4.Conclusion The therapeutic key to C-type fractures of distal femur lies in accurate anatomic reduction,right internal fixation based on the fracture style,simultaneous reparation of injured ligament and early knee mobilization.
