老年急性脑血管病并发高渗性非酮症糖尿病昏迷26例分析

目的：探讨老年急性脑血管病并发高渗性非酮症糖尿病昏迷(简称高渗性昏迷)的临床特点。方法：对26例急性脑血管病并发高渗性昏迷住院患者进行前瞻性研究，并做统计学分析。结果：老年急性脑血管病并发高渗性昏迷早期糖尿病症状不明显，而主要表现为精神神经症状，易漏诊。病情严重程度与血糖、血钠、渗透压升高成正相关，病死率高。结论：对伴有精神神经系统症状而无糖尿病病史及表现的老年患者应急查血电解质及血糖．以早期诊断，早期治疗，及时纠正高血糖、高血钠、高渗透压，可降低老年急性脑血管病并高渗性昏迷的病死率。

Analysis of nonketotic hyperosmolar coma of diabetes in 26 cases of aged patients with acute cerebralvascular diseases

Objective:To explore the clinical characteristic of nonketotic hyperosmolar coma of diabetes(NHCD) in the aged patients with acute cerebralvascular diseases.Methods:26 aged patients of acute cerebralvascular disease complicated with NHCD were studied and the data were analyzed statistically.Results:The symptoms of diabetes were not obvious in aged patients of acute cerebralvascular diseases.The main manifestations were the nervous and mental symptoms.The severe degree was correlated to blood glucose,sodium and plasma osmotic pressure with a high mortality.Conclusion:The electrolyte and blood glucose should be detected in order to correct hyperglycemia,high osmotic pressure and decrease the mortality of acute cerebralvascular diseases complicated with NHCD in the aged patients.
