膝关节滑膜皱襞综合征中MRI的使用价值

目的：探讨膝关节滑膜皱襞综合征中MRI的使用价值。方法方便收集2011年2月—2015年2月该院诊断为膝关节滑膜皱襞综合征的400例患者,入院后给予关节镜与MRI检查。分析(1)MRI下不同滑膜皱襞宽度与诊断阳性率的关系。(2)MRI与关节镜检查结果。(3)滑膜皱襞的出现与左右膝侧的关系。结果(1)MRI皱襞宽度0~10 mm者、10~20 mm者、>20 mm者的滑膜皱襞综合征阳性率结果比较差异有统计学意义(P<0.05)。(2)794膝中,共有169膝接受关节镜检查,与“金标准”关节镜对比,MRI灵敏度为92.6%,特异度为94.6%。(3)左右膝侧滑膜皱襞出现的阳性率分别为60.9%、56.6%,结果比较差异无统计学意义(P>0.05)。结论该研究认为MRI对诊断膝关节滑膜皱襞综合征灵敏度及特异度高,MRI下不同皱襞宽度患者的滑膜皱襞综合征阳性率差异有统计学意义。

Use Value of MRI in Synovial Plica Syndrome

Objective To discuss the use value of MRI in synovial plica syndrome. Methods 400 cases of patients with synovial plica syndrome diagnosed in our hospital from February 2011 to February 2015 were collected and were given arthroscopy and MRI examination after admission, and the correlation between the different synovial plica widths under MRI and the diagnostic positive rate, arthroscopy and MRI examination results and the correlation between the occurrence of synovial plica and left and right knee sides were analyzed. Results (1) There was a difference in the positive rate of synovial plica syndrome between the patients whose synovial plica width was between 0-10mm, 10-20mm and >20mm under MRI. (2)There were 169 knees receiving arthroscopy examination of the 794 knees, and the sensitivity and specificity of MRI were respectively 92.6% and 94.6% compared with those of "gold standard" arthroscopy.(3)There was no difference in the posi-tive rate of synovial plica between the left and right knee sides(60.9% vs 56.6%)(P>0.05). Conclusion The study shows that the sensitivity and specificity of MRI in diagnosis of synovial plica syndrome are high, and there is a difference in the posi-tive rate of synovial plica syndrome between patients with different plica widths under MRI.
