地佐辛复合罗哌卡因对臂丛神经阻滞麻醉下手术的应激反应

摘    要:目的 观察地佐辛复合罗哌卡因对臂丛神经阻滞麻醉下手术的应激反应.方法 上肢手术60例,按随机数字表法分为对照组和观察组,每组30例,对照组采用0.5%罗哌卡因行臂丛神经阻滞麻醉;观察组采用0.5%罗哌卡因复合地佐辛10 mg的混合液行臂丛神经阻滞麻醉.麻醉前30 min (T0),术中1 h(T1),手术后6 h(T2)观察血糖(blood glucose,Glu)水平、血浆皮质醇(cortisol,Cor)水平.结果 T0时2组患者Clu、Cor的浓度比较差异无统计学意义(P>0.05);与T0时比较,T1、T2时2组患者Clu、Cor的浓度开始增加(P<0.05);T1、T2时观察组患者Clu、Cor的浓度均低于对照组(P<0.05).结论 地佐辛复合罗哌卡因较单纯罗哌卡因对臂丛神经阻滞麻醉下手术的应激反应少,无明显不良反应.

Abstract:Objective To observe the stress response to surgery under brachial plexus block anesthesia with a combination of dezocine and ropivaeaine. Methods Sixty patients received brachial plexus block anesthesia with 0.5% ropivacaine(control group, n=30) or in combination with l0 mg dez- oeine for upper extremity surgery(observation group, n=30). Levels of blood glucose (Glu) and plasma cortisol (Cor)were measured 30 minutes before anesthesia(T0),1 hour after the beginning of operation (T1),and 6 hours after operation(T2). Results There were no significant differences in levels of Glu and Cor between the two group at T0(P〉0.05). Compared with To, levels of Glu and Cor increased at T1 and T2 in both groups(P〈0.05). Compared with control group, levels of Glu and Cot decreased in obse- rvation group at T1 and T2(P〈0.05). Conclusion The combination of dezocine and ropivacaine results in fewer adverse reactions and less stress response to surgery than ropivacaine alone.
