术后硬膜外自控镇痛、连续镇痛及肌肉注射镇痛的对比分析

目的：探讨手术后镇痛效果最佳、副作用最小的镇痛方法。方法：回顾分析我院2006年1月-2008年12月择期手术900例术后镇痛患者的临床资料,随机分为3组,术后硬膜外自控镇痛（I组）、硬膜外连续镇痛（II组）及肌肉注射镇痛（III组）。结果：术后VAS评分Ⅰ组明显优于Ⅱ组和Ⅲ组,SS评分12小时以后Ⅱ组优于Ⅰ组,但是Ⅱ组的布比卡因和芬太尼的用量明显多于I组,I组仍优于Ⅲ组。结论：硬膜外自控镇痛具有明显的镇痛优点和连续稳定的镇痛效果,避免波峰及波谷现象,用药量较小,副作用少。

Comparative analysis of pain relief on postoperative epidural controlled analgesia, continuous analgesia and intramuscular analgesia

Objective: To explore the best analgesic effect and minimal side effects of postoperative analgesia in three different ways. Methods:From January 2006 to December 2008,900 elective surgery patients with postoperative pain were randomly divided into epidural controlled analgesia (Ⅰ group),continuous epidural analgesia (Ⅱ group) and intramuscular analgesia (Ⅲ group) in which clinical data were comparatively analysed. Results: VAS score in Ⅰgroup was better than that in Ⅱ and Ⅲ groups. SS score after 12 hours in Ⅱ group was better than that in Ⅰgroup in which SS score after 12 hours inⅠgroup was still better than that in Ⅲ group,but the amount of bupivacaine and fentanyl were more than Ⅰ and Ⅲ groups. Conclusion: Epidural analgesia has low dosage,few side effects and continuously stable analgesic effect and avoiding peaks and troughs phenomenon.
