母牛分枝杆菌菌苗在治疗结核性胸膜炎中的作用

目的　评价母牛分支杆菌菌苗 (微卡 )治疗结核性胸膜炎 (tuberculous pleurisy,TP)的效果。方法　随机将 70例 TP患者分为治疗组 (35例 )和对照组 (35例 )。两组均采用统一抗结核化疗方案 2 HAL Z/6 HL2 及胸穿抽液 ,治疗组肌肉注射微卡每周 1次共 3个月 ,对照组口服强的松 6～ 8周。观察两组病例胸水吸收情况及胸膜肥厚情况 ,观察微卡副作用。结果　治疗组胸膜肥厚率 2 .9% ,对照组 2 2 .9% ,有显著性差异 (P<0 .0 5 ) ;治疗组平均胸水吸收时间 19.5天 ,对照组 2 8.2天 ,有高度显著性差异 (P<0 .0 1) ;微卡副作用有 :局部硬结 1例 ,低热 1例。结论　微卡能明显加快胸水吸收 ,降低胸膜肥厚率 ,用药安全 ,用于治疗 TP是一种较好的药物

The therapeutic effect of mycobacterium vaccae vaccine on the treatment of tuberculous pleurisy

Objective To evaluate the therapeutic effect of Mycobacterium vaccae (M. vaccae) vaccine on the treatment of tuberculous pleurisy. Methods Seventy tuberculouis pleurisy patients were randomly divided into two groups: study group (n=35) and controls group (n=35). All the patients of both groups were treated using chemotherapy with a regimen of 2 HALZ/6HL 2 and aspirating pleural effusion. For the study group, the patients were also given intramuscular injections of M. vaccae vaccine once a week for three months. For the controls, the patients took prednisone for six to eight weeks. Results The rates of pleural thickening were 2.9% in the study group and 22.9% in the control group (P < 0.05). The mean days of absorption of pleural effusion was 19.5 for the study group and 28.2 for the control group (P < 0.01). The side effects of M. vaccae vaccine were indicated as local induration (1 case) and slight fever (1 case).Conclusion M. vaccae vaccine is an effective and safe drug to reduce pleural thickening in treating patients with tuberculous pleurisy.
