终末期糖尿病肾病血液透析危险因素分析及护理对策

目的探讨影响终末期糖尿病肾病(DN)血液透析患者生活质量和长期存活的各种危险因素及护理对策。方法选择38例终末期DN血液透析患者和同期38例相同各年龄段非糖尿病血液透析患者,分别观察血生化改变、存活率、内瘘使用情况及并发症。结果DN组1年和2年存活率分别为76.3%和60.5%;主要并发症是高血压、心力衰竭、心律失常、感染、透析时低血压,与非DN组比较有显著性差异。结论终末期DN患者较非糖尿病患者应更早进行血透;加强护理,减少并发症发生是提高生活质量和长期存活的关键。

Risk Factors and Nursing Strategies for Patients with End Stage Diabetic Nephropathy Undergoing Hemodialysis

Objective To investigate the risk factors that influence the quality of life and survival time of the end stage diabetic nephropathy (ESDN) patients undergoing hemodialysis, as well as the nursing measures. Methods 38 cases of hemodialysis patients with ESDN in the DN group were observed on the changes of blood biochemical indexes, survival rate, status of internal fistula and complications. 38 cases of hemodialysis patients without DN with the same age at the same period in the non-DN group were taken as the control. Results The one-year-survival rate and two-year-survival rate in the DN group were 76.3% and 60.5% retrospectively. The main complications in patients with DN were hypertension, heart failure, cardiac arrhythmia, infections, intradialytic hypotension. There were significantly differences between the two groups. Conclusions Early dialysis and appropriate nursing measures play an important role in improving the quality of life and prolonging survival time in hemodialysis patients with DN.
