达英-35联合二甲双胍治疗多囊卵巢综合征后促排卵效果观察

目的:探讨应用达英-35联合二甲双胍治疗多囊卵巢综合征(PCOS)伴胰岛素抵抗(IR)不孕症的临床效果。方法:将64例PCOS伴有IR不孕患者随机分为两组,A组32例(单独应用达英-35治疗),B组32例(应用达英-35联合二甲双胍治疗),两组患者均治疗3个周期后促排卵。观察两组患者用药前后体重、体重指数、血糖、胰岛素水平、血清性激素及促排卵结局。结果:两组患者治疗后血清睾酮较治疗前明显降低,B组患者空腹胰岛素、胰岛素曲线下面积等显著下降;治疗后B组妊娠率较A组增高,周期取消率、卵巢过度刺激综合征(OHSS)发生率较A组降低,差异均有统计学意义(P<0.05)。结论:PCOS伴有IR不孕患者应用达英-35联合二甲双胍治疗,可明显改善内分泌失控状态,在此基础上促排卵,可提高妊娠率。

Clinical effects of Diane-35 combined with metformin among infertile women with polycystic ovary syndrome

Objective： To study the clinical effects of Diane-35 combined with metformin on ovulation induction in the infertile women suffering form insulin resistance（IR） with polycystic ovary syndrome（PCOS）.Methods： Sixty-four infertile women suffering from IR with PCOS were randomly divided into two groups： group A（n=32,only treated with Diane-35） and group B（n=32,treated with Diane-35 combined with metformin）.Three treatment cycles were followed by inducing ovulation.Body weight,body mass index（BMI）,serum glucose concentration,fasting insulin concentration,serum sex-hormone concentration,and induced ovulation outcome were evaluated in all women before and after treatment.Results： After the treatment,serum testosterone concentration was significantly decreased in both groups;the fasting insulin concentration,area under the insulin curve decreased significantly in group B;the clinical pregnant rate was higher,and the cycle cancellation rate and the incidence of ovarian hyperstimulation syndrome were lower in Group B（P all 0.05）.Conclusion： The endocrine state can be obviously improved in the infertile women suffering from IR with PCOS by treatment of Diane-35 combined with metformin.On this basis,the clinical pregnant rate could be increased after ovulation induction.
