脉通散外敷联合利伐沙班治疗恶性肿瘤患者下肢深静脉血栓形成的临床观察

目的观察脉通散外敷联合利伐沙班治疗恶性肿瘤患者下肢深静脉血栓形成(DVT)的临床疗效。方法将76例恶性肿瘤下肢DVT患者按照随机数字表法分为2组。对照组38例采用利伐沙班口服治疗;治疗组38例在对照组治疗基础上联合脉通散外敷治疗。2组疗程均为4周。比较2组治疗前后双下肢大、小腿周径差及凝血指标D-二聚体(D-D)、纤维蛋白原(FIB)、凝血酶原时间(PT)、活化部分凝血酶原时间(APTT)]变化,并统计疗效。结果治疗组总有效率81.58%(31/38),对照组总有效率60.53%(23/38),治疗组疗效优于对照组(P<0.05)。治疗后2组双下肢大、小腿周径差均降低(P<0.05),治疗组治疗后双下肢大、小腿周径差均小于对照组(P<0.05)。治疗后2组D-D、FIB均降低(P<0.05),PT、APTT均延长(P<0.05);治疗组治疗后D-D、FIB均低于对照组(P<0.05),PT、APTT均长于对照组(P<0.05)。结论脉通散外敷联合利伐沙班治疗恶性肿瘤下肢DVT患者疗效确切,能明显改善患者的凝血功能指标,且安全性高。

Clinical observation of Maitongsan external application combined with rivaroxaban in the treatment of lower extremity deep venous thrombosis in patients with malignant tumor

Objective To observe the effect of Maitongsan external application combined with rivaroxaban in the treatment of lower extremity deep venous thrombosis(DVT)in patients with malignant tumor.Methods 76 patients with lower extremity DVT were randomly divided into two groups according to the random number table method.38 cases in control group were treated by rivaroxaban.38 cases in treatment group were treated by Maitongsan external application on the basis of the control group.Both groups were treated for four weeks.Before and after treatment,the difference of peripheral diameter of legs and coagulation indexesD-Dimer(D-D),fibrinogen(FIB),prothrombin time(PT),activated partial prothrombin time(APTT)]were compared between the two groups.The efficacy of the two groups was counted.Results The total effective rate was 81.58%(31/38)in treatment group and 60.53%(23/38)in control group.The clinical curative effect of the treatment group was better than that of the control group(P<0.05).After treatment,the difference of peripheral diameter of big and small legs in the two groups were decreased(P<0.05),and the difference in the treatment group was lower than that in the control group(P<0.05).After treatment,D-D and FIB were decreased(P<0.05),PT and APTT were prolonged(P<0.05).The D-D and FIB in the treatment group were lower than those in the control group(P<0.05),PT and APTT were longer than those in the control group(P<0.05).Conclusion Maitongsan external application combined with rivaroxaban is effective in the treatment of lower extremity DVT patients with malignant tumors,which can significantly improve the coagulation function of patients,and has high safety.
