左氧氟沙星与盐酸氨溴索联合治疗对重症肺炎老年患者免疫功能、肺功能和血气指标的影响研究

摘    要:目的:探究左氧氟沙星与盐酸氨溴索联合治疗对重症肺炎老年患者免疫功能、肺功能和血气指标的影响,为临床治疗老年重症肺炎提供新方案.方法:选取我院收治的92例重症肺炎患者,按随机数字表法将患者分为观察组(47例)和对照组(45例),对照组患者给予左氧氟沙星治疗,观察组患者在对照组基础上给予盐酸氨溴索治疗,检测两组患者治疗前后免疫细胞、肺功能指标及血气指标变化.结果:与治疗前相比,治疗后两组患者免疫细胞(CD3+、CD4+和CD4+/CD8+)、肺功能指标用力肺活量(FVC)、最高呼气流速(PEF)、第1秒钟最大呼气容积(FEV1)以及平均最大呼气流量(MMEF)和血气指标动脉血氧分压(PaO2)、血氧饱和度(SpO2)、氧合指标(PaO2/FiO2)显著升高,二氧化碳分压(PaCO2)水平显著降低,而CD8+水平变化不明显;治疗后观察组老年患者免疫细胞(CD3+、CD4+和CD4+/CD8+)、肺功能指标(FVC、PEF、FEV1及MMEF)和血气指标(PaO2、SpO2、PaO2/FiO2)显著高于治疗后对照组,PaCO2水平显著低于治疗后对照组,而CD8+水平变化不明显;结论:左氧氟沙星与盐酸氨溴索的联合治疗提高了重症肺炎患者的免疫功能、改善了患者呼吸状况、增加了患者血氧水平.

Abstract:Objective To observe the influence of levofloxacin combined with ambroxol hydrochloride in the treatment of elderly patients with severe pneumonia then offer new scheme for severe pneumonia treatment.Methods A total of92 elderly patients with severe pneumonia were selected and randomly divided into the observation group (47 patients) and the control group (45 patients). The control group patients were treated with levofloxacin. The observation group patients were treated with levofloxacin combined with ambroxol hydrochloride. Before and after treatment, the changes of Immune cell, lung function and blood gas index were analyzed. The related mechanism of combined therapy was analyzed.Results Compared with before treat-ment, immune cells (CD3+, CD4+ and CD4+/CD8+), lung function index forced vital capacity FVC (FVC), peak expiratory flow rate (PEF), first seconds maximum expiratory volume (FEV1) and the average peak expiratory flow (MMEF) and blood indexes of arterial oxygen partial pressure (PaO2), oxygen saturation (SpO2), the oxygenation index (PaO2/FiO2) of two groups after the treatment increased significantly, but partial pressure of carbon dioxide (PaCO2) decreased significantly, the changes of CD8+ was not obvious. After treatment, the immune cells (CD3+, CD4+ and CD4+/CD8+), pulmonary function indexes (FVC, PEF, FEV1 and MMEF) and blood gas indexes (PaO2, SpO2, PaO2/FiO2) in the observation group were significantly higher than those in the control group, PaCO2 decreased significantly, the changes of CD8+ was not obvious. Conclusion Combined treat-ment of levofloxacin and ambroxol can improve the immune function of patients with severe pneumonia, improve the respiratory status of patients, and increase blood oxygen levels in patients.
