综合性介入治疗在中晚期肝癌中的临床应用

目的：探讨综合性介入治疗在中晚期肝癌中的临床应用价值。方法：单纯肝动脉栓塞化疗和综合性介入治疗两组对比，观察治疗前后肿瘤大小、血供、门静脉癌栓及甲胎蛋白（AFP）的变化，声像图改变与临床疗效的关系。结果：两组治疗前后，肿瘤血供消失和减少率，肿瘤缩小率，门静脉癌栓消失和减少率的差异具有显著意义。肿瘤血供的减少或消失多与肿瘤的缩小、声像图回声的均匀增强及AFP的下降相一致，是病变良好转归的表现，反之亦然。结论：综合性介入治疗对预防肝癌转移、延迟复发、改善预后有肯定意义，明显延长病人带瘤生存期，提高了生活质量。

THE CLINICAL APPLICATION OF COMPREHENSIVE INTERVENTIONAL TREATMENT IN THE MIDDLE AND ADVANCED STAGE OF HEPATIC CARCINOMA

Objective:To evaluate the clinical significance of comprehensive interventional treatment in the middle and advanced stage of hepatic carcinoma.Methods:Simple transhepatic arterial chemo-embolization and comprehensive interventional treatment were contrasted,observed the change of blood supply and size to tumor,the sonographic characteristics,portal vein tumor thrombosis,the levels of AFP and the clinical efficacy before and after treatment.Results:Two group's differences show significant meaning in the changes of blood supply and size to tumor and portal vein tumor thrombosis before and after treatment.Reduction or disappearance of the blood flow corresponds to reduction of the tumor,equable increase of echogenicity and decrease of AFP levels.The changes can be considered as signs of favourable prognosis,and vice versa.Conclusion:Comprehensive interventional treatment is of certain value in preventing metastasis and relapse,improving prognosis and prolonging the period of survival to advanced hepatic carcinoma.
