Abstract:Objective To investigate the value of intravoxel incoherent motion(IVIM)combined with diffusion weighted imaging(DWI)in evaluating the efficacy of treatment for primary lung cancer.Methods From February 2016 to February 2019,58 patients with advanced non-small cell lung cancer(NSCLC)who received platinum based standard first-line chemotherapy at Peking Union Medical College,Chinese Academy of Medical Sciences were selected.All patients were examined by CT and MRI before the treatment,and the efficacy was evaluated by CT at 2 weeks after the treatment.The patients were divided into an effective group(20 patients)and an ineffective group(38 patients)according to the change in maximum diameter of tumors on CT.Apparent diffusion coefficient(ADC),diffusion coefficient(D),pseudodiffusion coefficient(Dp),perfusion fraction(f),distributed diffusion coefficient(DDC)and diffusion heterogeneity index(α)were measured.The receiver operating characteristic(ROC)curve was used to evaluate the predictive efficiency of the parameters with statistical significance.Results The ADC and D value were lower in the effective group than in the ineffective group(all P<0.05),and there was no significant difference in the rest parameters between the two groups(P>0.05).The sensitivity and the specificity of D and ADC value were 89.5%vs 65.0%and 65.0%vs 85.0%%,respectively for the prediction of efficacy of first-line chemotherapy for NSCLC.The diagnostic threshold was 0.599×10-3mm2/s and 1.233×10-3mm2/s,respectively.Conclusion The parameters of D and ADC value,derived from IVIM and DWI,can potentially evaluate the efficacy of treatment for primary lung cancer.These results are expected to provide more imaging evidences for individualized treatment of advanced NSCLC.
