神经生物反馈治疗难治性癫痫的临床研究

摘    要:目的:观察神经生物反馈(又称脑电图生物反馈,EEG biofeedback,EBF)治疗难治性癫痫的临床疗效。方法:采用随机、单盲、对照方法。试验组进行EBF训练来强化Ca导联12~15Hz的感觉运动节律(SMR)而同时抑制θ节律,共30次为一疗程。对照组患者进行空白对照。对试验前后的临床发作情况和脑电图进行观察。结果:共56例完成了30次的EBF训练,其中36例为试验组患者,20例为对照组。经EBF治疗后,试验组癫痫发作明显减少,总有效率为69%,对照组中总有效率为25%。试验组重度异常脑电图所占的比例由47%降至28%,总好转率为56%(20/36)。结论:难治性癫痫患者进行EBF训练,在强化C4导联SMR节律的同时抑制θ节律,可以减少癫痫发作。

Abstract:Objective: To evaluate the effect of neurofeedback (EEG biofeed back, EBF) as the treatment of intractable epilepsy. Methods: A prospective controlled single-blind trial was conducted with EEG biofeedback(EBF) training sixteen-nine, subjects were randomly allocated to training with either a reinforcement SMR (sensorimotor rhythm,12-15Hz) and reduction rhythm protocol or to a non-neuro- feedback control group. Subjects were assessed prior and subsequent to the training process on quantitative EEG and seizure outcome. Subjects (both groups were refractory patients with epilepsy) continued to take their primary anti-epileptic drugs(AEDs) and EBF training was as an add-on therapy. Results: Fifty-six patients fulfilled total 30 sessions of training. In EBF training group, the effectiveness was 69% ,and it was 25% in the control group. The EEG waves were improved significantly after EBF. Conclusion: EBF training of SMR/θ band leads to significant reduction of seizure ons'et and improvement of epileptic symptoms.
