经尿道膀胱肿瘤电切联合表柔比星膀胱灌注治疗非肌层浸润性膀胱癌的临床研究

目的评价膀胱肿瘤电切术(TURBT)+表柔比星膀胱内灌注治疗非肌层浸润性膀胱癌的疗效和安全性。方法对42例非肌层浸润性膀胱癌患者行经尿道膀胱肿瘤电切术(TURBT),术后定期应用EPI(30mg/40ml)膀胱内灌注,每次灌注药物在膀胱内保留1小时,每周1次,共8次,以后每月1次,共1年。结果 42例患者均获得12~24个月随访,无肿瘤复发38例(90.5%),复发4例。未见有全身性药物不良副作用,有4例膀胱灌注后出现轻度膀胱刺激症状。结论膀胱肿瘤电切术联合表柔比星膀胱灌注治疗非肌层浸润性膀胱癌效果明显,微创,安全,术后复发率低,患者耐受好,副作用小。

Clinical Research of Transurethral Resection of Bladder Tumors Combined with Bladder Irrigation of Epirubicin for the Treatment of Non-muscle Invasive Bladder Cancer

Objective To evaluate the effects and safety of the treatment of non-muscle invasive bladder cancer by transurethral resection of bladder tumor(TURBT) combined with bladder irrigation of epirubicin.Methods 42 patients with non-muscle invasive bladder cancer were treated by transurethral resection of bladder tumor(TURBT).Bladder irrigation of epirubicin(30 mg/40 ml) started at the first week after surgical operation,which was retained in the bladder for one hour,once a week,total of 8 times.Then once a month,total of one year.Results 42 patients were followed-up,ranging from 12 to 24 months,38 cases(90.5%) had no tumor recurrence and 4 cases had tumor recurrence.There were no systemic adverse effects,only 4 cases had the symptoms of mild bladder irritation.Conclusion The treatment of non-muscle invasive bladder cancer by transurethral resection of bladder tumor(TURBT) combined with bladder irrigation of epirubicin has revealed that there is obviously effect,minimally wound,safety,less recurrence rate,good tolerance by patient and fewer side effects.
