高血压患者应用他汀类药物对脉搏波传导速度及炎症因子影响的研究

目的探讨高血压患者降压治疗同时联合应用他汀类药物能否更好地控制动脉粥样硬化的进展。方法按入选标准入选405例高血压患者,随机分为他汀组和对照组。他汀组降压治疗同时予以瑞舒伐他汀钙10 mg/d,对照组予以单纯降压治疗。研究周期为18个月,记录两组患者IL-6(白介素-6)、MMP-9(基质金属蛋白酶9)、血脂系列基线检测、6个月、12个月、18个月数值及臂踝脉搏波传递速度(ba PWV)的基线检查和18个月结果,通过对两组患者观察项目不同随访时间点与基线数值变化进行组内、组间比较分析。结果他汀组与基线比较三酰甘油(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、IL-6、MMP-9降低,HDL-C与基线值比较升高,左侧ba PWV有下降,差异均有统计学意义。对照组各项指标与基线比较无统计学差异。6、12、18个月时TG、TC、LDL-C、IL-6、MMP-9他汀组低于对照组,HDL-C高于对照组,均有统计学意义。结论高血压患者降压治疗的同时联用他汀类药物显示出良好的调脂,降低MMP-9、IL-6和改善动脉弹性的作用。

Influences of statins on pulse wave velocity and inflammatory factors in hypertensive patients

Objective To investigate the effect of anti-hypertension treatment combining statins in controlling the development of atherosclerosis in hypertensive patients.Methods The patients （n=405） were chosen according to inclusion criteria and then randomly divided into statins group and control group. The statins group was given rosuvastatin calcium （10 mg/d） and control group was only given anti-hypertension treatment. A study circle was 18 m. The changes of interleukin-6 （IL-6）, matrix metalloproteinase-9 （MMP-9）, and baseline detections of blood fat and pulse wave velocity （PWV） were recorded in 2 groups at time-points of 6 m, 12 m and 18 m. The changes of observation indexes and baseline values were compared and analyzed within or between 2 groups at different time-points.Results In statins group, the levels of triglyceride （TG）, low-density lipoprotein-cholesterol （LDL-C）, IL-6 and MMP-9 decreased, and level of high-density lipoprotein-cholesterol （HDL-C） increased compared with baseline values, and left baPWV decreased with statistical significance. In control group, the difference between all indexes and baseline values had no statistical significance. At time-points of 6 m, 12 m and 18 m, the levels of TG, CHOL, LDL-C, IL-6 and MMP-9 were lower, and HDL-C level was higher in statins group than those in control group with statistical significance.Conclusion Anti-hypertension treatment combining statins has effects of adjusting lipid, reducing MMP9 and IL-6, and improving arterial elasticity in hypertensive patients.
