中老年人颈动脉粥样硬化的超声评价及相关性探讨

【目的】探讨中老年人颈动脉粥样硬化的超声结构和脉压差、血脂、血糖、血尿酸的相关关系。【方法】采用百胜DU8型彩色超声诊断仪,5～12MHz探头,将368例患者按颈动脉硬化程度分为内膜中膜增厚组、斑块形成组和管腔狭窄组,分别测定其与对照组的总胆固醇、甘油三脂、血糖、血尿酸、脉压差,观察颈动脉粥样硬化程度与血脂、血糖、血尿酸、脉压差的关系。【结果】斑块形成组和管腔狭窄组血脂、血糖、血尿酸、脉压差显著高于内膜中膜增厚组和对照组（P〈0.05）。Logistic多元回归分析发现,颈动脉粥样硬化程度与总胆固醇、甘油三脂、血糖、血尿酸、脉压差呈显著正相关（P〈0.05）。【结论】总胆固醇、甘油三脂、血糖、血尿酸、脉压差是颈动脉粥样硬化的独立危险因素。及时检测患者血脂、血糖、血尿酸、脉压差和颈动脉粥样硬化情况,评价病变程度,对疾病的早期防治具有重要的价值。

Ultrasongrahic evaluation and relationship study in elderly patients with carotid atherosclerosis

[Objective] To study the relationship among pulse pressure,blood-lipid,fasting blood glucose（FBG）,blood uric acid（UA）and the ultrasonic construction in elderly patients with atherosclerosis of carotid artery.[Methods] The instruments used Technos MPX（DU8）color Doppler ultrasonography diagnosis system with L5/12 MHz linear array transducer.368 patients with carotid atherosclerosis were divided into intima-media thickness group,plaque development group and vessel stenosis group.Concentration of total cholesterol（TC）,triglycerides（TG）,FBG,blood UA and pulse pressure（PP）were measured in 368 patients and 56 cases of control group.The relationship between the degree of carotid artery atherosclerosis and the abovementioned parameters was observed.[Results] The concentration of blood-lipid,FBG,blood UA and pulse pressure were significantly higher in plaque development group and vessel stenosis group（P0.05）,and they were obviously higher in intima-media thickness group than in control group（P0.05）.the result of Logistic regression analysis indicated there were positive correlation among blood-lipid,FBG,blood UA,pulse pressure and carotid artery atherosclerosis（P0.05）.[Conclusions]Blood-lipid,FBG,blood UA and pulse pressure were the independent risk factors of carotid artery atherosclerosis.The concentration of blood lipid,FBG,blood UA,pulse pressure and the degress of carotid artery atherosclerosis in patients should be measured in time,the severity of disease should be evaluated early,all of these are of importance for the clinical prevention and treatment of disease.
