血必净联合乌司他丁治疗脓毒血症36例疗效分析

目的：探讨研究血必净联合乌司他丁治疗脓毒血症的临床疗效。方法：将符合标准的36例脓毒血症患者的临床资料，根据患者意愿分成观察组（20例）和对照组（16例），观察组给予血必净联合乌司他丁治疗，对照组给予血必净治疗。测定与比较治疗前后血气变化情况、肺组织中白介素-6（IL-6）与肿瘤坏死因子-α（TNF—α）的浓度、血清降钙素原（PCT）浓度以及T淋巴细胞亚群。结果：在治疗两周后，观察组PH值与PaO2值均大于对照组，PaCO2值性小于对照组，差异均具有统计学意义（P〈0．05）；观察组IL-6值与TNF—α值均小于对照组，PCT值小于对照组，CD4^＋、CD8^＋与CD4^＋／CD8^＋均高于对照组，差异均具有统计学意义（P〈0．05）。结论：血必净联合乌司他丁对于脓毒血症的治疗有较好的疗效，该疗效与其抗感染能力以及改善免疫功能有关，值得临床推广使用。

Clinical Study of Xuebijing injection and ulinastatin in treatment of 36 patients with sepsis

Objective. To integrative clinical efficacy of Xuebijing injection combined with ulinasta- tin in the treatment of sepsis. Methods. Totally 36 patients with sepsis were divided into observed group （20 cases） and control group （16 cases）, patients in observation group received Xuebijing injection com- bined with ulinastatin, while others received the treatment of Xuebijing injection only. Detected and com- pared the blood gas changes, the concentration of IL-6 and INF-α in lung, the concentration of PCT, and T lymphocyte subsets between observation group and control group before and after treatment. Results. After the two-week treatment, the pH and PaO2 in the observation group were significantly higher than that in control group （P〈0. 05）, and PaCO2 was significantly lower than that in control group （P〈 0.05）. The IL-6 and INF-α in observation group were significantly lower than that in control group（P〈0.05）. The PCT in observation group was significantly lower than that in control group（P〈0.05）. The CD4^＋ , CD8^＋ and CD4^＋/CD8^＋ in observation group were significantly higher than that in control group （P〈0.05）. Conclusion： Xuebijing injection combined with ulinastatin shows good curative effects on sep- sis. This curative effect has a close relationship with their ability of infection resistance and immune func- tion improvement. It is worth to be promoted in clinic.
