联合检测血清甘氨酰脯氨酸二肽氨基肽酶与胆碱酯酶对胃癌的诊断价值

目的:探讨血清中甘氨酰脯氨酸二肽氨基肽酶(GPDA)与胆碱酯酶活力测定对胃癌的诊断价值.方法:采用速率法联合检测76例经内镜活检及外科手术病理证实的胃癌患者血清中的GPDA及胆碱酯酶含量(选50例正常人血清作对照),并对胃镜检查及外科手术中癌肿生长情况进行分析.结果:76例胃癌患者中,血清GPDA值较平均值明显下降者60例(79%),与未下降者比较差异有显著性(P<0.05);胆碱酯酶值较平均值明显下降者65例(86%),与未下降者比较差异有显著性(P<0.01);66%胃癌患者血清中GPDA及胆碱酯酶含量同时明显降低,与未同时下降者比较差异有显著性(P<0.05);而正常对照组几乎正常或仅轻度降低.胃镜检查中发现,血清中GPDA及胆碱酯酶明显降低的患者癌肿多浸润超过一个解剖分区,且手术中发现,胃癌浸润深度多已超过浆膜层,并多有局部及(或)远处淋巴结转移.结论:血清中GPDA及胆碱酯酶可作为胃癌的标志物,对胃癌的筛查及胃癌患者预后的判定都有着重要的临床意义.

Diagnostic value of glycylproline dipeptidylamino-peptidase combined with cholinesterase in the sera to gastric cancer

Objective:To study on diagnositic value by measuring the activities of glycylproline dipeptidylaminopeptidase (GPDA) combined with cholinesterase in serum to gastric cacinoma. Methods:Using velocity technigue to detect the level of GPDA and cholinesterase in sera of 76 patients with gastric cancer certificated by endoscope biopsy and surgical pathology (In addition,there were 50 sera from healthy persons as control), and analysed by combining with carcinoma growth conditions during endoscopy and operation. Results: GPDA index of sera among with 76 patients gastric cancer, was obvious descent (79%). In comparision with control,the differeme was significant ( P <0.05); cholinesterase index in 65 people was obvious descent (86%). It has shown difference between descent and undescent cases ( P <0.01). 66% of cancer patients has low level of both GPDA and cholinesterase .In comparision with control,the difference was significant ( P <0.05). But control bundle scarcely has obvious descent. With endoscopy it was found that patients with obvious lower index of GPDA and cholinesterase whose carcinoma always infiltrate beyond a anatomical subregion and that the depth of infiltration always went beyond serosa, accompanied with local and/or distant lymph nodes metastases. Conclusion: GPDA combined with cholinesterase as the marker of gastric cancer has clinically diagnostic value for gastric cancer screening and prognostic evaluation of gastric cancer patients.
