新疆哈萨克族与汉族高血压患者血浆硫化氢水平比较及相关性研究

摘    要:[目的]比较新疆哈萨克族(哈族)、汉族原发性高血压(EH)患者血浆硫化氢(H2S)水平并分析其相关因素.[方法]哈族133人及同期在本院体检中心体检的汉族130人,根据血压水平,分为哈、汉EH组,哈、汉对照组,测量血压、身高、体质量、腰围(WC)、臀围,检测血浆H2S、胰岛素(FINS)、血糖(FPG).计算体质量指数(BMI)、腰臀比(WHR)、体内脂肪百分比(BF%)、胰岛素抵抗指数(IRI)等并进行比较.[结果]①哈、汉EH组血浆H2S水平明显低于哈、汉对照组(P〈0.01),哈、汉EH组间无显著性差异(P〉0.05);对照组中汉族血浆H2S水平高于哈族(P〈0.05).②哈、汉EH组中,血浆H2S水平与收缩压(SBP)、WC、BMI、IRI呈负相关(P〈0.05或0.01),哈族EH组亦与WHR、FINS呈负相关(P〈0.01).③多元逐步回归分析示,在哈族EH组中,SBP、WC、BMI、IRI是影响血浆H2S的独立因素;在汉族EH组中,SBP、BMI、IRI是影响血浆H2S的独立因素;在哈、汉族对照组中,BMI是影响血浆H2S的独立因素.[结论]哈、汉EH患者血浆H2S水平降低,且与血压、肥胖、胰岛素抵抗存在相关性,在健康人群中,汉族血浆H2S水平高于哈族,血浆H2S水平降低可能是哈族EH发病率较高的原因之一.

Comparison of Plasma Hydrogen Sulfide Levels in Patients with Essential Hypertension Between Kazakh and Han in Xinjiang and Their Correlations

Abstract:[Objective]To compare plasma hydrogen sulfide(H2S)levels in the patients with essential hypertension(EH)of Xinjiang Kazakh and Han, and analyze correlation factors. [Methods]A total of 133 Kazakhs and 130 Hans cases that came from physical examination center of our hospital in the same period were selected. According to blood pressure, they were divided into EH group of Kazakh and Han, control group of Kazakh and Han. Blood pressure, body height, weight, waist circumference(WC) and hip circumference were measured. . The plasma HzS, fasting insulin(FINS) and fasting plasma glucose (FPG) were detected, and body mass index(BMI), waist-hip ratio (WHR), body fat percentage(BF%) and insulin resistance index(IRI) were calculated. [Results]The plasma H2 S level in the EH groups of Kazakh and Han was significantly lower than that in the control groups( P 〈0.01). There was no significant difference in the EH groups between Kazakh and Han ( P 0.05), but there was significant, difference in the control group between Kazakh and Han ( P 〈0.05). In the EH group of Kazakh and Han, the plasma H2 S was negatively' correlated with systolic blood pressure(SBP), WC, BMI and IRI( P 〈0.05 or 0.01). In the EH group of Kazakh, the plasma H2S was also negatively correlated with FINS( P〈0.01). Multiple regression analysis showed that SBP, WC, BMI and IRI were independent factors to affect the H2S in the EH group of Kazakh. SBP, BMI and IRI were independent factors to affect the H2 S in the EH group of Han, BMI was an independent factor to affect the H2 S in control group of Kazakh and Han. [Conclusion]The plasma H2 S level is decreased in the patients with EH of Kazakh and Han, and correlated with SBP, obesity and insulin resistance. In the healthy group, the plasma H2S level of Han is higher than that of Kazakh, which is one of possible reasons that cause high incidence rate of EH.
