多层螺旋CT对结核活动性判断的价值

目的探讨肺结核MSCT基本征像及MSCT对结核活动性判断的临床应用价值。方法分组统计继发性肺结核初治、好转期及非活动性肺结核MSCT基本征像特点，归纳出对结核活动性判断有价值征像。结果70例3组患者中，继发性肺结核初治患者MSCT主要征像有小叶中心结节及腺结节征（21／25）、小叶内网线结构（18／25）、树芽征（22／25）、磨玻影（15／25）、液体支气管征（8／25）和斑片影（24／25），前5种征像与B组好转期及C组非活动性肺结核比较有统计学意义。其中斑片影三组出现率均较高，无显著性差异，索条影有统计学意义（P〈0．01）。结论继发性肺结核各期MSCT征像有一定特征性，MSCT对结核活动性判断有重要临床应用价值。

The value of multi-slice CT in the judgement of the activity of tubercle

Objective To investigate the diagnosis and clinical value of reactiveness of tubercle with multi-slice CT. Methods Statistical analysis was conducted on the basical MSCT sign and features of pulmonary tuberculosis with initial treatment, resolving stage and inactive tuberculosis by group, to summarize the significance of the diagnosis of reaction of tubercle. Results In the 70 cases, MSCT sign feature of pulmonary tuberculosis with initial treatment was ＂sign of nodus of lobule center＂ （21/25 ）, reticle within lobule （ 18/25 ）, ＂bud sign＂ （ 22/25 ）, ＂grinding glass sign＂ （ 15/25 ）, ＂fluid bronchus sign＂ （ 8/25 ）, and ＂patching shadowing＂ （ 24/25 ）. The first 5 signs had statistical significances comparing the resolving stage with inactive tuberculosis groups. MSCT sign feature of the group B was ＂patching shadowing＂ （22/25）, strip shadowing （ 21/25 ）, ＂sign of nodus of lobule center＂ （15/25 ）. MSCT sign feature of the group C was strip shadowing （18/20） , ＂patching shadowing＂（16/20） , and calcification （11/20）. The ＂patching shadowing＂ had the highest frequency, which showed no significant difference, while the strip shadowing showed statistical significance（ P 〈 0. 01 ）. Conclusion MSCT sign of pulmonary tuberculosis is markable and MSCT is important in the diagnosis.
