膀胱内翻性乳头状瘤22例报告

目的：探讨提高对膀胱内翻性乳头状瘤的发病特点及其诊治水平。方法：25例膀胱内翻性乳头状瘤术前均行B超、膀胱镜检查，22例行静脉尿路造影(IVU)检查，14例行尿细胞学检查，17例术前行膀胱镜活检。21例行经尿道膀胱肿瘤切除术(TURBT)，4例行开放性手术。结果：全部病例术后病理诊断均为内翻性乳头状瘤，其中2例恶变，病理分型：小梁型6例，腺型2例。尿细胞学未找到癌细胞；21例行TURBT的患者，随访6个月～2年无复发。结论：膀胱内翻性乳头状瘤是非浸润性泌尿上皮良性肿瘤，好发于膀胱颈及三角区，肿瘤较小，单发，有蒂，可恶变，很少复发，诊断依赖于膀胱镜检及术后病理检查。TURBT是理想的治疗方法。

Inverted papilloma of the urinary bladder (Report of 22 cases)

Purpose:To improve the diagnosis and treatment of inverted papilloma of urinary bladder. Methods:Ultrasound and cystoscopy were done in all patients. Intravenous urogram (IVU) were done in 22 cases, cytology in 14 cases and biopsy under cystoscope in 17 cases before surgery. Twenty one patients were treated with transurethral resection of bladder tumor (TURBT) and the rest 4 cases treated by open local resection. Results:Inverted papilloma was diagnosed by postoperative pathology for all patients, 2 cases of whom were found to have concomitant malignant transformation. Further pathological typing was carried out in 8 patients, which included 2 cases of glandular type and 6 cases of trabecular type. Cytology showed no cancer cell. All 21 patients treated with TURBT were followed up 6 months to 2 years and no recurrence was seen. Conclusions:Inverted papilloma is non invasive benign urothelial tumor. It predominantly occurs in the bladder trigone and neck region as small solid and pedunculated lesion with malignant potential and rare recurrence. TURBt is the ideal treatment choice for this discase.
