摘    要:目的 探讨Helicobacterpylori(Hp)相关性胃炎中Hp密度、cagA基因对胃黏膜iNOS表达的影响。 方法 免疫组化技术对iNOS进行定位、定量测定 ,用双因素设计分析。结果 iNOS的表达主要见于浸润在固有层的炎性细胞的胞浆中。Hp密度对iNOS表达有影响 (P <0 .0 0 0 1) ,cagA 和cagA-组iNOS表达有差别 (P =0 .0 0 0 4 ) ,且二者对iNOS的表达有交互作用 (P =0 .0 0 4 6 )。在cagA 层内 ,各密度组间iNOS的表达有差别 (P <0 .0 0 0 1) ,随着密度的增高 ,iNOS表达水平增高 :3级较 2级组iNOS表达水平明显增高 (16 1.35± 6 5 .13)vs(119.5 3± 2 8.12 ) ,P =0 .0 0 13],2级较 1级组iNOS表达水平高 ,且有统计学意义 (119.5 3± 2 8.12 )vs (78.4 0± 2 7.18) ,P =0 .0 2 36 ];而在cagA-层内 ,随着密度的增高 ,各密度组间iNOS的表达水平有增高趋势 ,无统计学差异 (6 0 .70± 13.0 7)vs(75 .75± 5 .32 )vs (76 .5 0± 19.17) ,P =0 .2 10 8]。在同一密度层内 ,cagA 与cagA-组iNOS表达有差异 :1级层内二者差别无显著性 (78.4 0± 2 7.18)vs (6 0 .70± 13.0 7) ,P =0 .894 8],2级层内cagA 组较cagA-组iNOS表达增高 (119.5 3± 2 8.12 )vs (75 .75± 5 .32 ) ,P =0 .0 0 0 1],3级层内cagA 组iNOS表达水平显著

Abstract:Objective To study the influence of cagA and density of Helicobacter pylori(Hp)on expression levels of iNOS in gastric mucosa. Methods iNOS was determined by immunohistochemistory. Results iNOS protein was mainly localized in the cytoplasm of inflammatory cells filtrating into the lamina propria. The level iNOS with cagA patients were significantly higher than that of cagA - ones(P<0.05). Higher density grade enhanced the expression of iNOS ,especially for the cagA layer. Conclusion The infection with H.pylori higher density and cagA strains enhance the level of iNOS, higher grade of H.pylori density have more effect on the iNOS expression, especially in the layer of cagA .
