氯氮平和利培酮对催乳素、甲状腺素的影响

摘    要:目的:比较氯氮平、利培酮对精神分裂症患者血清催乳素(PRL)、三碘甲状腺原氨酸(T3)、甲状腺素(T4)的影响。方法:将68例精神分裂症患者随机分为氯氮平组和利培酮组,治疗6周。用磁酶免疫法在治疗前后分别测定血清PRL、T3、T4水平。结果:氯氮平组治疗后血清PRL、T3、T4均增高,治疗前后差异有显著性;但血清PRL、T3、T4的改变无性别的差异;利培酮血清PRL治疗后较治疗前高,T3、T4较治疗前低,差异均有显著性,也无性别的差异。结论:氯氮平和利培酮治疗均明显增加血清催乳素水平,利培酮增加幅度更大。氯氮平增加T3、T4水平,而利培酮降低T3、T4水平。

Abstract:Objective:To study the effects of risperidone and clozapine on the serum prolactin, T 3 and T 4 levels. Method:Sixty eight patients who met the criteria of CCMD 2 R for schizophrenia were randomly divided into two subgroups receiving double blinded treatment of risperidone and clozapine respectively for 6 weeks. Serum PRL, T 3, T 4 were detected by monoclonal immunoradiometric assay. Results:In clozapine group, PRL, T 3 and T 4 levels increased significantly after the treatment, but the variation of serum PRL, T 3 and T 4 had no significant differences between male and female patients. In risperidone group, serum PRL increased whereas serum T 3 and T 4 levels decreased after the treatment. There was significant differences between before and after the treatment, but the variation had no significant differences between male and female patients again. PRL, T 3 and T 4 levels significantly varied before and after the treatment in both groups. Conclusion: Both clozapine and risperidone increases the serum PRL concentrations significantly, but risperidone can produce a higher increase; Clozapine increases serum T 3 and T 4levels, but risperidone decreases serum T 3 and T 4 levels.
