临床药师干预围术期患者预防用抗菌药物情况分析

目的:调查我院临床药师干预前、后Ⅰ类切口手术围术期预防用抗菌药物情况。方法:采用回顾性方法,随机抽取我院2011年4-6月(非干预组)和2011年9月-11月(干预组)Ⅰ类切口手术患者的出院病历,对预防用抗菌药物的合理性进行比较分析。结果:通过临床药师对围术期预防用抗菌药物的干预,围术期预防用药在药物选择、用法用量、给药时机等方面均得到明显提高,合理率分别由干预前的5.71%、68.57%、11.43%提升至干预后的75.90%、95.18%、75.90%(P<0.05);人均抗菌药物费用由814.6元下降至433.5元(P<0.05);人均抗菌药物应用时间由4.07d下降至3.65d。结论:通过制定规范的围术期预防用抗菌药物干预措施,临床药师参与临床合理用药,可显著提高围术期合理用药比例,提高医院的合理用药水平。

Analysis of Pharmacists Intervening in Prophylactic Application of Antibacterials during Perioperative Period

OBJECTIVE：To investigate the perioperative prophylactic application of antibacterials for typeⅠincision operation before and after clinical pharmacists’ intervention.METHODS：In retrospective study,the discharged medical records of patients underwent typeⅠincision operation were randomly collected from our hospital during Apr.-Jun.in 2011（non-intervention group） and Sept.-Nov.in 2011（intervention group）.RESULTS：The drug selection,usage and dosage,administration timing had been improved significantly by clinical pharmacists’ intervention during perioperative period.The reasonable rates were raised from 5.71%,68.57%,11.43% to 75.90%,95.18%,75.90%,respectively（P〈0.05）;the average cost of antibacterials per capita was reduced from 814.6 yuan to 433.5 yuan（P〈0.05） and the medication duration of antibacterials per capita was reduced from 4.07 d to 3.65 d.CONCLUSIONS：Through developing standard intervention measures for perioperative prophylactic application of antibacterials,clinical pharmacists are involved in rational use of drugs in the clinic,which can improve the proportion of rational drug use during perioperative period and rational use of antibiotics in hospital.
