我院重症监护室抗菌药物应用与铜绿假单胞菌耐药的相关性研究

摘    要:目的:分析我院重症监护室抗菌药物应用与铜绿假单胞菌耐药之间的相关性,为临床合理应用抗菌药物提供参考。方法:采用回顾性调查研究方法,以半年为一个时间段,统计重症监护室2008-2010年住院患者抗菌药物应用情况及同期铜绿假单胞菌的药敏试验结果;采用SPSS17.0软件对每一时间段内铜绿假单胞菌对各种常用抗菌药物的耐药率与各类抗菌药物的总用药频度(DDDs)之间进行Pearson相关分析。结果:第3代头孢菌素类、第4代头孢菌素类、喹诺酮类、青霉素类+酶抑制剂这几类抗菌药物的应用与铜绿假单胞菌对哌拉西林/他唑巴坦、头孢吡肟、阿米卡星、环丙沙星、左氧氟沙星的耐药率之间具有相关性。结论:抗菌药物的应用与细菌耐药之间存在一定的相关性,应更加注意规范抗菌药物的应用,特别是限制那些高潜在耐药的抗菌药物的应用。

Abstract:OBJECTIVE: To analyze the correlation between antibacterials use and drug resistance of Pseudomonas aeruginosa in ICU of our hospital, and to provide theory base for reasonable use of antibiotics in the clinic. METHODS : Through retrospective survey research methods with six months as a period of time, the utilization of antibiotics and drug sensitivity test of P aeruginosa in ICU during 2008 -2010 were analyzed statistically. Pearson relationship of drug resistance of P. aeruginosa for each period to common correlation with DDDs of antibiotics was analyzed by using SPSS 17.0 statistical software. RESULTS: The utilization of third and fourth generation cephalosporins, quinolones, penicillins+inhibitors was positively correlated with drug resistance of P. ae- ruginosa to piperacillin/tazobactam, cefepime, amikacin, ciprofloxacin gatifloxacin, levofloxacin. CONCLUSION: The utilization of correlation and bacterial resistance exist a certain correlation between them. We should pay more attention to standardized rational use of correlation, particularly limit the use of antibiotics with high potential drug resistance.
